摘要
目的探讨低压低氧环境对失血性休克大鼠氧合状态的影响。方法将30只清洁大鼠随机分为失血性休克未进低压低氧舱组(A组)、未休克进低压低氧舱组(B组)、失血性休克进低压低氧舱组(C组),各10只。采用容控法制备失血性休克模型,分析各组不同时间点酸碱度(pH)、氧分压(PO_(2))、二氧化碳分压(PCO_(2))、血氧饱和度(SaO_(2))、乳酸(LAC)、碳酸氢根离子(HCO_(3)^(-))、碱剩余(BE)、血糖(Glu)、血红蛋白(Hb)水平变化情况。结果A、C组T1时pH值及SaO_(2)、LAC、HCO_(3)^(-)、BE水平比较,差异有统计学意义(P<0.05),而PO_(2)、PCO_(2)、Glu、Hb水平比较,差异无统计学意义(P>0.05)。A、C组T3时pH值及HCO_(3)^(-)、BE水平比较,差异有统计学意义(P<0.05),而PO_(2)、PCO_(2)、SaO_(2)、LAC、Glu、Hb水平比较,差异无统计学意义(P>0.05)。B、C组T1时pH值及LAC、HCO_(3)^(-)、BE、Hb水平比较,差异有统计学意义(P<0.05),而PO_(2)、PCO_(2)、SaO_(2)、Glu水平比较,差异无统计学意义(P>0.05)。B、C组T3时pH值及LAC、HCO_(3)^(-)、BE、Hb水平比较,差异有统计学意义(P<0.05),而PO_(2)、PCO_(2)、SaO_(2)、Glu水平比较,差异无统计学意义(P>0.05)。C组不同时间点pH值及LAC、BE水平比较,差异有统计学意义(P<0.05)。结论低压低氧环境可加重失血性休克大鼠酸中毒程度。但随着时间的延长,失血性休克大鼠因本身较强的代偿能力,其酸中毒程度并未发生持续恶化。
Objective To investigate the effect of hypobaric hypoxic environment on oxygenation in rats with hemorrhagic shock.Methods Thirty clean rats were randomly divided into three groups with 10 in each group:hemorrhagic shock group without entering the hypobaric hypoxia chamber(group A),non shock group without entering the hypobaric hypoxia chamber(group B),and hemorrhagic shock group with entering the hypobaric hypoxia chamber(group C).Hemorrhagic shock model was prepared by volume control method,and the changes of pH,PO_(2),PCO_(2),SaO_(2),LAC,HCO_(3)^(-),BE,Glu and Hb were analyzed at different time points in each group.Results There were significant differences in the pH value and the levels of SaO_(2),LAC,HCO_(3)^(-)and BE at T1 between the group A and C(P<0.05),while there were no significant differences in the levels of PO_(2),PCO_(2),Glu and Hb(P>0.05).There were significant differences in the pH value and the levels of HCO_(3)^(-)and BE at T3 between the group A and C(P<0.05),while there were no significant differences in the levels of PO_(2),PCO_(2),SaO_(2),LAC,Glu and Hb(P>0.05).There were significant differences in the pH value and the levels of LAC,HCO_(3)^(-),BE and Hb at T1 between the group B and C(P<0.05),while there were no significant differences in the levels of PO_(2),PCO_(2),SaO_(2)and Glu(P>0.05).There were significant differences in the pH value and the levels of LAC,HCO_(3)^(-),BE,Hb at T3 between the group B and C(P<0.05),while there were no significant differences in the levels of PO_(2),PCO_(2),SaO_(2)and Glu(P>0.05).There were significant differences in the pH value and the levels of LAC and BE at different time points in the group C(P<0.05).Conclusion Hypobaric hypoxic can aggravate acidosis in rats with hemorrhagic shock.However,with the extension of time,the degree of acidosis in rats with hemorrhagic shock does not deteriorate continuously due to their strong compensatory ability.
