基于胜任力的整形外科专科医师规范化培训多中心自评结果分析

Analysis of multi-center self-evaluation results of standardized training for plastic surgery specialists based on competency

目的了解我国整形外科专科医师培训现况,并在此基础上进行教学方法以及考核方式的改进。方法2022年3—4月,北京大学第三医院成形外科对我国8所拥有整形外科专科培养资质的医院专培阶段的医师发放基于胜任力要求自行设计的调查问卷,对其结果进行统计分析。结果不同年级专科医师自我评分差异无统计学意义(P>0.05)。不同医院科研素质(P=0.025)、专业知识(P=0.025)、临床技能(P=0.017)差异具有统计学意义。不同学历下科研素质水平、专业知识、临床技能差异均有统计学意义(P<0.05)。担任过总住院医师的科研素质、专业知识、临床技能水平显著高于未担任总住院医师者(P<0.05)。专科医师对于整形外科手术技能的掌握仍存在较大问题。结论整形外科专科医师需要分层差异化的培养。不同医院在早期培养时侧重内容不同,因此医师在科研能力和临床技能方面有较大差异。目前国内专科医师在临床技能掌握方面仍有不足,对于整形外科经典手术的掌握与国外存在一定差距。国内专科医师培养方案以及考核体系还需不断完善,从而提升专科医师岗位胜任力,培养出更适应现代医学需求的医教研全面发展高素质医学人才。

Objective To understand the current situation of the training of plastic surgery specialists in China and to improve the teaching methods and assessment methods on this basis.Methods From March to April 2022,questionnaires were distributed to physicians at the specialized training stage in 9 hospitals with plastic surgery qualifications in China.The results of the questionnaires were statistically analyzed.Results There was no significant difference in self-ratings of specialized training physicians in different grades(P>0.05).There were significant differences in scientific research quality(P=0.025),professional knowledge(P=0.025)and clinical skills(P=0.017)in different hospitals.There were significant differences in the level of scientific research quality,professional knowledge and clinical skills under different educational backgrounds(P<0.05).The scientific research quality,professional knowledge and clinical skills of those who have served as residents are significantly higher than those who have not served as residents(P<0.05).Master the skills of plastic surgery were still the big problems for specialists.Conclusion Plastic surgery specialists need stratified and differentiated training.Different hospitals have different emphasis in the early training,so have great differences in scientific research ability and clinical skills.At present,domestic specialists are still insufficient in the mastery of clinical skills,and there is a certain gap between the mastery of classical plastic surgery and foreign countries.The domestic specialist training program and assessment system still need to be continuously improved,so as to enhance the post competence of specialist physicians and cultivate high-quality medical talents who are more suitable for the needs of modern medicine.
