血细胞比容与血压的相关性研究

Study on the correlation between hematocrit and blood pressure

目的:分析血细胞比容(HCT)与血压(BP)的相关性。方法:根据排除标准初步筛选出2020年6月至2022年6月在湖南省人民医院和湘雅博爱康复医院住院部及体检中心完善24 h动态血压监测(ABPM)的原发性高血压(EH)患者450例,同期选取在体检中心体检的BP正常者150例。其中男性306例,女性294例。依据《2018 ESC/ESH高血压防治指南》中EH的分组标准,将600例受试者分为四组,即1级高血压组、2级高血压组、3级高血压组和非高血压组,每组各150例。比较四组的性别、年龄、体质指数(BMI)、吸烟史、饮酒史等一般资料,同时比较其HCT、血肌酐(Scr)、尿酸(UA)、低密度脂蛋白胆固醇(LDL-C)等生化指标,并通过Pearson相关性分析总结HCT与BP之间的关系。结果:(1)四组的年龄、HCT、BMI、LDL-C、UA比较有统计学差异(P<0.05)。(2)HCT与24 h平均收缩压(mSBP)、24 h平均舒张压(mDBP)、24 h平均动脉压(MAP)呈正相关(P<0.05)。结论:HCT与BP相关,随着HCT的增加,24 hmSBP、24 hmDBP、24 hMAP会明显增高。

Objective:To analyze the correlation between hematocrit(HCT)and blood pressure(BP).Methods:According to the exclusion criteria,450 patients with essential hypertension(EH)were preliminarily selected from June 2020 to June 2022 in the inpatient department and physical examination center of Hunan People’s Hospital and Xiangya Boai Rehabilitation Hospital,and 150 patients with normal BP were selected from physical examination center during the same period.There were 306 males and 294 females.According to the EH grouping standard in 2018 ESC/ESH Guidelines for Hypertension Prevention and Treatment,600 subjects were divided into four groups,namely Grade 1 hypertension group,grade 2 hypertension group,grade 3 hypertension group and nonhypertension group,with 150 cases in each group.Sex,age,body mass index(BMI),smoking history,drinking history and other general data of the four groups were compared.Meanwhile,HCT,serum creatinine(Scr),uric acid(UA),low density lipoprotein cholesterol(LDL-C)and other biochemical indicators were compared.The relationship between HCT and BP was summarized by Pearson correlation analysis.Results:(1)There were significant differences in age,HCT,BMI,LDL-C and UA among the four groups(P<0.05).(2)HCT was positively correlated with 24 h mean systolic blood pressure(mSBP),24 h mean diastolic blood pressure(mDBP)and 24 h mean arterial pressure(MAP)(P<0.05).Conclusion:HCT is correlated with BP.With the increase of HCT,24 hmSBP,24 hmDBP and 24 hMAP will increase significantly.
