标准化临床路径在单孔胸腔镜下肺叶切除术患者围手术期护理中的应用

Effects of standardized clinical pathway in perioperative nursing of patients undergoing pulmonary lobectomy under single-port thoracoscope

目的探讨标准化临床路径模式在单孔胸腔镜下肺叶切除术患者围手术期护理中的应用效果。方法以便利抽样法选取2019年4月—2020年4月在郑州人民医院进行单孔胸腔镜手术的142例患者为对照组,选取2020年5月—2021年5月手术的142例患者为观察组。对照组围手术期采用常规护理,观察组构建标准化临床路径表进行围手术期护理。比较两组患者术后相关指标、疼痛评分、并发症发生率、护理满意度。结果观察组患者的术后排气时间、拔管时间、下床活动时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。观察组术后12、24、48、72 h的疼痛评分均低于对照组(P<0.05)。观察组术后的并发症总发生率低于对照组,护理满意度高于对照组,差异均有统计学意义(P<0.05)。结论单孔胸腔镜手术患者围手术期护理中应用标准化临床路径可有效降低患者疼痛水平,减少并发症,促进患者术后康复。

Objective To explore the effect of standardized clinical pathway in perioperative nursing of patients undergoing pulmonary lobectomy under single-port thoracoscope.Methods A control group of 142 patients who underwent single-port thoracoscopic surgery in Zhengzhou People's Hospital from April 2019 to April 2020 were selected as the control group,and 142 patients who underwent surgery from May 2020 to May 2021 were selected as the observation group.The control group received routine nursing during the perioperative period,while the observation group carried out perioperative nursing by constructing a standardized clinical pathway.The postoperative related indexes,pain score,complication and nursing satisfaction were compared between the two groups.Results The time of exhaust,extubation,ambulation and hospitalization of patients in the observation group was shorter than that in the control group,with statistically significant differences(P<0.05).The pain scores of the observation group at 12,24,48,72 h after operation were lower than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,and the nursing satisfaction was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions The standardized clinical pathway in perioperative nursing of patients undergoing single-port thoracoscopic surgery can effectively reduce the pain and complication of patients,and promote postoperative rehabilitation.
