育龄期妇科癌症患者自我感受负担的影响因素及与共同冗思的相关性

Influencing factors of self-perceived burden in gynecological cancer patients of childbearing age and the correlation with co-ruminative thinking

目的了解育龄期妇科癌症患者自我感受负担和共同冗思现状,分析两者之间的相关性,为提供精准护理干预提供参考依据。方法2021年6—12月选取山东省某三甲医院育龄期妇科癌症患者148例为调查对象,采用共同冗思问卷、癌症患者自我感受负担量表(SPBS-CP)进行调查,分析两者之间的相关性。结果育龄期妇科癌症患者自我感受负担总分52.14±17.96分,共同冗思平均3.09±0.75分。单因素分析结果显示,共同冗思总分及理解性、关注消极感受维度与自我感受负担总分均呈正相关(P<0.05);关注消极感受维度分别与照顾负担、经济负担和家庭负担、治疗负担呈正相关(P<0.05)。多因素线性回归分析显示,共同冗思水平与育龄期妇科癌症患者自我感受负担呈正相关(P<0.05)。结论育龄期妇科癌症患者自我感受负担为中度水平,共同冗思程度较深。冗思程度可影响患者自我感受负担水平,医护人员和家庭可共同从改善共同冗思程度入手,针对不同疾病分期、治疗阶段来降低育龄期妇科癌症患者自我感受负担水平。

Objective To understand the status quo of self-perceived burden and co-ruminative thinking in gynecological cancer patients of childbearing age, and to analyze the correlation between them, to provide reference for accurate nursing intervention. Methods From June to December 2021, 148 gynecological cancer patients of childbearing age in a Class Ⅲ hospital in Shandong Province were selected as the investigation objects, and the correlation between them was analyzed by using the common thinking questionnaire and the Selfperceived patients(SPBS-CP). Results The total score of the patients with gynecological cancer in childbearing age was 52.14±17.96 points, and the average co-ruminative thought was 3.09±0.75 points. Univariate analysis showed that the total score of co-ruminative thinking, comprehensibility and focus on negative perception were positively correlated with the total score of self-perceived burden(P<0.05). The dimensions of focus negative feelings were positively correlated with caregiving burden, economic burden, family burden and treatment burden respectively(P<0.05). Multivariate linear regression analysis showed that the co-ruminative thinking level was positively correlated with the self-perceived burden of gynecological cancer patients at childbearing age(P<0.05).Conclusion Gynecological cancer patients of childbearing age have a moderate level of self-perceived burden and a deep degree of co-ruminative thinking. The degree of redundancy could af fect the level of patients’ perceived burden. Medical staf f and families can jointly improve the degree of common redundancy and reduce the level of patients’ perceived burden according to dif ferent disease stages and treatment stages.
