Summary of best evidence on discharge planning for stroke patients

目的:检索、评价和综合脑卒中病人出院计划的最佳证据,为制订脑卒中病人出院计划方案提供循证依据。方法:系统检索美国国立指南网(NGC)、英国国家卫生与照护优化研究所(NICE)、加拿大安大略省注册护士协会(RNAO)、MedLine、PubMed、Web of Science、中国知网、中国生物医学文献数据库(CBM)、万方等数据库资源,检索时限为自建库至2021年5月1日,采用Joanna Briggs Institute(JBI)循证卫生保健中心的文献评价标准和证据分级系统,对各类研究进行质量评价及证据级别评定。结果:共纳入15篇文献,包括9篇指南、5篇专家共识、1篇标准。共总结出44条证据,包括多学科团队合作、入院后24~48 h的早期评估、出院前评估、出院后随访及相关支持、病人及其照顾者健康教育内容、相关工作人员培训6个方面。结论:总结现有脑卒中病人出院计划的最佳证据,为形成脑卒中出院计划方案提供科学系统的理论主干,为后续实现有效的出院过渡提供参考。

Objective:To search,evaluate and synthesize the best evidence of discharge planning for stroke patients,and provide evidence-based basis for formulating discharge plan for stroke patients.Methods:Systematic searches of database resources such as National Guideline Clearinghouse(NGC),National Institute of Health and Clinical Excellence(NICE),Registered Nurses′Association of Ontario(RNAO),MedLine,PubMed,Web of Science,CNKI,CBM,WanFang Database,etc.The retrieval time was from inception to May 1,2021.The JBI Evidence-Based Health Care Center′s literature evaluation criteria and evidence grading system were used to evaluate the quality and evidence level of various studies.Results:A total of 15 papers were included,including 9 guidelines,5 expert consensus papers,and 1 standard paper.A total of 44 pieces of evidence were summarized,including six aspects of multidisciplinary teamwork,early assessment 24-48 h after admission,pre-discharge assessment,post-discharge follow-up and support,health education for patients and their caregivers,and relevant staff training.Conclusion:The best evidence of the existing discharge planning for stroke patients was summarized,which provides a scientific and systematic theoretical backbone for the formation of the discharge planning for stroke patients,and provides a reference for the subsequent effective discharge transition.
