济南市基层医疗卫生机构医生离职意向及影响因素分析

Turnover Intention and Its Influencing Factors among Doctors of Primary Healthcare Institutes in Jinan

摘要 目的:了解济南市基层医疗卫生机构医生离职意向现状,并分析其影响因素。方法:2020年6—9月采用分层整群随机抽样的方法,在济南市6个区选择30家基层医疗卫生机构的426名医生为研究对象,采用自编问卷调查医生的基本信息、工作状况、职业倦怠、工作满意度和离职意向。离职意向评分大于3分为有离职意向。结果:426名医生离职意向平均得分为(1.94±0.989)分,有40人(9.39%)有离职意向。二分类logistic回归分析结果显示,年龄、工作压力、工作满意度和职业倦怠的情绪耗竭程度是基层医生离职意向的影响因素。结论:济南市基层医疗卫生机构医生离职意向评分较低,但仍有9.39%的医生有离职意向。建议进一步减轻基层医疗卫生机构医生工作压力,降低职业倦怠水平,提升工作满意度,从而降低其离职意向。 OBJECTIVE To investigate turnover intension and analysis its influencing factors among the doctors in primary care institutions in Jinan.METHODS From June to September in 2020,by using cluster sampling,426 doctors from 30 primary health care institutions in six districts and counties of Jinan were selected and invested by a self-developed questionnaire consisting general information,working status,burnout,job satisfaction and turnover intention.RESULTS The average score of the turnover intention of 426 doctors was 1.94±0.989 and more than 3 points of interviews accounted for 9.39%.Univariate analysis showed that the prevalence of turnover intention differed significantly by age,income,working press,emotional exhaustion,depersonalization,low personal accomplishment and job satisfaction.Stepwise binary Logistic regression analysis showed that age,work pressure,emotional exhaustion of burnout and job satisfaction were the influence factors for the turnover intention.CONCLUSION The turnover intention level of doctors in Jinan primary healthcare institutions was relatively low.The associated factors were age,work pressure,emotional exhaustion,and the doctors’job satisfaction.Actions such as improving the reward mechanism,alleviating the work pressure,reducing the job burnout and improving the job satisfaction were needed to relieve turnover intension.
