ICU护士临床决策能力的现状及其影响因素研究

Present status of clinical decision-making ability for ICU nurses and analysis on influencing factors

目的调查ICU护士临床决策能力现状及其影响因素,为提高ICU护士的临床决策能力提供依据。方法采用方便抽样法,选取172名ICU护士为研究对象,采用一般资料调查表、一般决策风格量表和护理临床决策测评工具进行调查,采用单因素分析、多元线性回归分析临床决策能力的影响因素。结果ICU护士临床决策能力总分为150.0(145.0,159.0)分。ICU护士的决策风格维度得分从高到低依次为理智型4.4(4.0,4.8)分、依赖型3.6(3.4,4.2)分、直觉-冲动型3.2(2.8,3.4)分和回避型2.8(2.2,3.2)分。多元线性回归分析结果显示,理智型决策风格是ICU护士临床决策能力的正向影响因素,回避型决策风格是ICU护士临床决策能力的负向影响因素(P均<0.001)。结论ICU护士的临床决策能力受理智型和回避型决策风格影响。护理管理者应加强针对性培训促使其理智型决策风格的形成,从而提高临床决策能力。

Objective To investigate the present status of clinical decision-making ability for ICU nurses and its influencing factors to provide reference for improving clinical decision-making ability for ICU nurses.Method Choose 172 ICU nurses by convenient sampling and investigate them by general information questionnaire,General Decision-Making Style and Clinical Decision Making in Nursing Scale.Analyze the influencing factors of clinical decision-making ability by single factor analysis and multiple linear regression.Result The total score on clinical decision-making ability for ICU nurses is 150.0(145.0,159.0).Scores on dimensions of decision-making style for ICU nurses from high to low are rational 4.4(4.0,4.8),dependent 3.6(3.4,4.2),intuitive-spontaneous 3.2(2.8,3.4)and avoidant 2.8(2.2,3.2).Multiple liner regression analysis shows that rational decision-making style is a positive influencing factor of clinical decision-making ability and avoidant decision-making style is a negative influencing factor of clinical decision-making ability(P<0.001)for ICU nurses.Conclusion Clinical decision-making ability for ICU nurses is influenced by rational and avoidant decision-making style.Nursing managers should enhance corresponding training to promote the formation of rational decision-making style so as to improve nurses’clinical decision-making ability.
