宫腔镜电切术联合孕激素对子宫内膜息肉患者性激素和复发率的影响

Effects of hysteroscopic resection combined with progesterone on sex hormones and recurrence rate in patients with endometrial polyps

目的 评价宫腔镜电切术联合孕激素对子宫内膜息肉(EP)患者性激素和复发率的影响。方法 选取48例EP患者,以随机数字表法分为治疗组与对照组,各24例,对照组行宫腔镜电切术,基于对照组,治疗组加用左炔诺孕酮治疗。比较两组EP复发率、性激素及子宫内膜厚度变化。结果 两组血清黄体酮激素、孕酮、雌二醇、卵泡刺激素水平治疗前后及组间比较,差异均无统计学意义(P>0.05)。治疗后两组子宫内膜厚度、孕激素受体和雌激素受体水平均降低,且治疗组低于对照组;治疗后3个月内治疗组复发率低于对照组,差异具有统计学意义(P<0.05)。结论 EP患者采用宫腔镜电切术联合孕激素治疗可有效促进患者的预后恢复,改善子宫内膜厚度,降低孕激素受体和雌激素受体水平,复发率较低。

Objective To evaluate the effects of hysteroscopic resection combined with progesterone on sex hormones and recurrence rate in patients with endometrial polyps(EP). Methods A total of 48 EP patients were selected and divided into a treatment group and a control group by the random number table method, 24 cases in each group, The patients in the control group were treated with hysteroscopic resection, and based on the control group, the treatment group was additionally treated with levonorgestrel. The EP recurrence rate, sex hormone changes and endometrial thickness changes were compared between the two groups. Results There was no significant difference in the levels of serum progesterone, progesterone, estradiol, and follicle-stimulating hormone between the two groups before and after treatment(P>0.05). Progesterone receptors and estrogen receptors of the two groups decreased after treatment, and the treatment group was lower than the control group;the recurrence rate of the treatment group was lower than that of the control group within 3 months after treatment, and the difference was statistically significant(P<0.05). Conclusion Hysteroscopic resection combined with progesterone therapy in patients with EP can effectively promote the recovery of the patient’s prognosis, improve endometrial thickness, reduce the levels of progesterone receptors and estrogen receptors, and have a lower recurrence rate.
