左心室基顶三角与年龄、身高、体重及心脏功能的关系

Relationship of left ventricular base‐apex triangle to age,height,body weight,and cardiac function

摘要 目的:通过超声心动图和电脑软件绘制左心室基顶三角,观察基顶三角各参数与年龄、身高、体重和心功能的关系并发掘其意义。方法:对连续性健康体检者104例行常规超声探查,采集标准心脏切面,利用自开发软件计算心尖四腔切面舒张和收缩末期三角边长、高、中线、面积,同时记录左室收缩与舒张功能相关参数。根据资料特征行Pearson或Spearman相关分析并计算相关系数(r)。结果:基顶三角的中线、高线、两侧边长相互之间均呈高度相关,舒张期r:0.93~0.99,收缩期r:0.89~0.99,P<0.01。舒张期和收缩期基顶三角的中线、高线、两侧边长与SV均呈低度相关,r:0.32~0.38,P<0.01。身高比年龄与左室长轴的相关程度更高,r:0.52~0.56,P<0.01。结论:本文提出了左室基顶三角的概念并发现:中线M是反映左室长轴综合变化的合理指标;身高比年龄与左室长轴相关度更高,解读相关结果时需更多考虑身高因素影响;室壁近似长度值(三角边长)不是一个能充分反映心室功能的指标,在心功能研究中宜直接测量室壁实际长度。 Objective:To construct a left ventrical base⁃apex triangle(a physiological triangle constituted by ventricular apex⁃septal and lateral mitral annulus)in healthy Chinese population by using echocar⁃diography and software,and to investigate the significance of the triangle′s values in relation to age,height,body weight,and cardiac function.Methods:A total of 104 healthy subjects were consecu⁃tively detected by color Doppler echocardiography.The base⁃apex triangular side lengths,height,midline,and area were measured in the four chamber view semi⁃automatically with a software,and the cardiac function parameters were recorded as well.Pearson or Spearman analyses were done on the datum characteristic to determine correlation coefficient(r).Results:High degree of positive correlation existed among the side lengths(AB and AC),height,and midline(diastolic r:0.93⁃0.99;sys⁃tolic r:0.89⁃0.99).The side lengths(AB and AC),height,and midline had low degree correlation with SV(r:0.32⁃0.38)and moderately with body height(r:0.52⁃0.56).P value was less than 0.01 in all the tests.Conclusion:Base⁃apex triangle was raised as composed of left ventricular apex,septal and lateral mitral annulus.The midline was found to be a reasonable parameter reflecting the whole ventricular length.The ventricular length is affected more by body height rather than age.Finally,the triangular lateral lengths could not well reflect the systolic function,and hence measurement of the real ventricular wall length would be preferable in the future work.
