侵蚀艾肯菌致胫骨慢性骨髓炎一例报告

Detection of an eikenella corrodensin patient with chronic osteomyelitis of tibia:a case report

侵蚀艾肯菌(Eikenella corrodens)属于HACEK细菌群,是人类黏膜表面正常菌群的一部分,大部分与牙周炎相关[1],也可引起上呼吸道、胸膜和肺、腹部、关节、骨骼等或其它部位的感染[2-5]。现报道笔者经治的1例侵蚀艾肯菌致胫骨骨髓炎病例。

Objective To explore the clinical diagnosis and treatment process of tibial osteomyelitis infected by eikenella corrodensin and to improve the understanding of it.Methods Clinical manifestation,diagnosis and treatment of an eikenella corrodensin patient with chronic osteomyelitis of tibia were reported.Results The patient was a 36-year-old male,who was admitted to the department of traumatic orthopedics of our center because of repeated infiltration and rupture of the anterior incision of the right tibia.Combined with clinical manifestations,X-ray image,laboratory examination and microbial culture results of sinus secretions,it was determined as chronic osteomyelitis caused by eikenella corrodens infection.After right leg lesion removal and vacuum aspiration,levofloxacin hydrochloride and imipenem had been applied for 2 weeks.The wound was clean and dry with no obvious abnormality in the sensation and peripheral blood supply of the right lower limb.The patient was discharged soon.Conclusions The infection of chronic osteomyelitis with eikenella corrodensin developed by open fracture is rare.We should be alert to microorganisms in clinic when the patient has a history of trauma and surgery.Microbiological laboratory should pay special attention to the separation of rare bacteria and fastidious bacteria,so as to provide references for clinical anti-infection treatment.
