摘要
目的:探究circ_0009910结肠癌患者肿瘤组织中表达及对预后的影响。方法:2014年1月—2019年1月,结肠癌117例患者术中的肿瘤组织和癌旁正常组织,采用实时荧光定量聚合酶链反应(q RT-PCR)技术测定肿瘤组织和癌旁正常组织中circ_0009910的相对表达量,测定并比较结肠癌患者癌组织中和癌周组织circ_0009910相对表达情况;比较并分析circ_0009910表达高低与临床特征之间的关系;电话随访所有患者1年,记录生存时间,SPSS Kaplan-Meier计算患者的生存函数;采用单因素和多因素COX回归分析结肠癌患者生存期的相关因素。结果:结肠癌患者癌组织中circ_0009910相对表达量明显高于癌周组织(P<0.05);circ_0009910的AUC为0.751,取最佳截断值时特异性为88%,敏感度为54.7%,约登指数为0.427,对结肠癌具有一定的诊断价值(P<0.05);circ_0009910的表达与年龄、性别、肿瘤大小、血管浸润无关,差异无统计学意义(P>0.05),而与结肠癌的临床分期、肿瘤分化程度、远处转移和淋巴结转移有关(P<0.05),结肠癌晚期(Ⅲ~Ⅳ期)、低分化肿瘤、有远处转移、有淋巴结转移的患者中,circ_0009910的相对表达量更高(P<0.05);高表达组患者的1年生存率为19.7%,低表达组的1年生存率为34.8%,circ_0009910表达水平高的患者生存率明显低于对照组(P<0.05);结肠癌的生存期与结肠癌的临床分期、分化程度、远处转移、淋巴结转移和circ_0009910有关(P<0.05),而与年龄、性别、肿瘤大小、血管浸润无关(P>0.05);COX多元性回归模型发现,临床分期、分化程度、远处转移、淋巴结转移和circ_0009910是结肠癌患者生存期的独立因素(P<0.05)。结论:circ_0009910的高表达与结肠癌患者的预后密切相关,可严重影响患者的生存期,对结肠癌患者的诊断具有重要意义。
Objective:To explore the expression in tumor tissues of patients with circ_0009910colon cancer and its effect on prognosis.Methods:From January 2014 to January 2019,117 patients with colon cancer diagnosed by Suzhou Science and Technology City Hospital affiliated to Nanjing Medical University and Nanjing traditional Chinese Medicine Hospital were selected as experimental subjects.The tumor tissues and adjacent normal tissues were collected,and the relative expression of circ_0009910 in tumor tissues and adjacent normal tissues was determined by real-time fluorescence quantitative polymerase chain reaction(q RT-PCR).To detect and compare the relative expression of circ_0009910 in cancerous and pericancerous tissues of patients with colon cancer.The relationship between the expression of circ_0009910 and clinical features was compared and analyzed;all patients were followed up by telephone for one year,the survival time was recorded,and the survival function was calculated by SPSSKaplan-Meier method;finally,univariate and multivariate COX regression was used to analyze the related factors of survival in patients with colon cancer.Results:The relative expression of circ_0009910 in colon cancer tissues was significantly higher than that in peri-cancer tissues(P<0.05).The AUC of circ_0009910 was 0.751,the specificity was 88%,the sensitivity was 54.7%,and the Yoden’s index was 0.427,which had certain diagnostic value for colon cancer.The expression of circ_0009910 was not related to age,sex,tumor size and vascular invasion,but related to clinical stage,tumor differentiation,distant metastasis and lymph node metastasis of colon cancer.It was found in patients with advanced colon cancer(stageⅢ-Ⅳ),poorly differentiated tumors,distant metastasis and lymph node metastasis.The relative expression of circ_0009910 was higher(P<0.05).The 1-year survival rate of the high expression group was19.7%,and the 1-year survival rate of the low expression group was 34.8%.The 1-year survival rate of the patients with high expression level was significantly lower than that of the control group(P<0.05).The survival time of colon cancer was related to clinical stage,differentiation,distant metastasis,lymph node metastasis and circ_0009910,but not to age,sex,tumor size and vascular invasion.Clinical stage,differentiation,distant metastasis,lymph node metastasis and circ_0009910 were included in COX multiple regression model as mixed factors.It was found that clinical stage,differentiation,distant metastasis,lymph node metastasis and circ_0009910 were independent factors of survival in patients with colon cancer.Conclusion:The high expression of circ_0009910 is closely related to the prognosis of patients with colon cancer,which can seriously affect the survival time of patients,and is of great significance in the diagnosis of patients with colon cancer.
