51岁女性胃母细胞瘤1例报道及文献复习

Gastroblastoma in a 51-year-old woman:a case report and literature review

目的对1例罕见程度较高的胃母细胞瘤病例进行报道,以期为该病的诊断、治疗及预后提供参考。方法回顾性报道江苏省中医院2021年9月16日收治的1例51岁女性胃母细胞瘤患者的诊疗过程,整理影像资料和病理资料等临床资料,并结合相关文献进行分析。结果患者主诉发现黑便半月余,入院后初步诊断为“胃间质瘤”,并行手术治疗。术后病理显示胃上皮间质混合性肿瘤,结合免疫组化标记结果倾向胃母细胞瘤。随访7个月未见复发及转移征象。结论结合已有文献报道,胃母细胞瘤的发病人群以年轻男性为主,好发部位为胃窦部,肿瘤生物学行为倾向惰性,多数病例预后良好。术后病理及免疫组织化学检查结果是目前明确诊断的唯一方法,手术为首选治疗方案。

Objective To report a rare case of gastroblastoma,and to provide reference for the diagnosis,treatment,and prognosis of the disease.Methods The diagnosis and treatment process of a 51-year-old female patient with gastroblastoma admitted to Jiangsu Provincial Hospital of Chinese Medicine on September 16,2021,was retrospectively reported,and clinical data such as imaging data and pathological data were sorted out and analyzed in combination with relevant literature.Results The patient complained chiefly of finding melena for more than half a month,and was initially diagnosis as“gastric stromal tumor”after admission,and underwent surgical treatment.Postoperative pathological results:gastric epithelial-mesenchymal mixed tumor,combined with immunohistochemical markers,tended to be gastroblastoma.There were no signs of recurrence and metastasis during the 7-month follow-up.Conclusion Combined with the existing literature reports,the incidence of gastroblastoma is mainly young men,and the predilection site is gastric antrum.The biological behavior of the tumor tends to be indolent,and the prognosis of most cases is favorable.Postoperative pathological and immunohistochemical examination results are the only methods for definite diagnosis at present,and surgery is the first choice for treatment.
