纤维支气管镜肺泡灌洗术联合红霉素注射液治疗儿童支原体肺炎的临床效果观察 被引量:2

摘要 目的:观察纤维支气管镜肺泡灌洗术联合红霉素注射液治疗儿童支原体肺炎的临床效果。方法:选取2019年5月-2022年5月江苏省扬州大学附属医院收治的100例支原体肺炎患儿,分为红霉素组与灌洗组,各50例。红霉素组给予红霉素注射液治疗,灌洗组给予纤维支气管镜肺泡灌洗术联合红霉素注射液治疗。对比两组症状消失时间、炎性因子水平、肺功能指标及临床疗效。结果:灌洗组咳嗽停止时间、啰音消失时间、体温复常时间均短于红霉素组,差异有统计学意义(P<0.05)。治疗前,两组红细胞沉降率(ESR)、降钙素原(PCT)、C反应蛋白(CRP)水平比较,差异无统计学意义(P>0.05);治疗后,灌洗组ESR、PCT、CRP水平均低于红霉素组,差异有统计学意义(P<0.05)。治疗前,两组用力肺活量(FVC)、第1 s用力呼气容积(FEV1)水平比较,差异无统计学意义(P>0.05);治疗后,灌洗组FVC、FEV1水平均高于红霉素组,差异有统计学意义(P<0.05)。灌洗组总有效率高于红霉素组,差异有统计学意义(P<0.05)。结论:纤维支气管镜肺泡灌洗术联合红霉素注射液治疗儿童支原体肺炎的临床效果显著,可有效缩短患儿临床症状持续时间,减少肺炎支原体数量,增强患儿免疫力,提高肺功能,加速患儿康复,值得临床应用并予以推广。 Objective:To observe the clinical effect of bronchofibroscope alveolar lavage combined with erythromycin injection in the treatment of mycoplasma pneumonia in children.Methods:A total of 100 children with mycoplasma pneumonia admitted to Affiliated Hospital of Yangzhou University in Jiangsu Province from May 2019 to May 2022 were selected and divided into the erythromycin group and the lavage group,with 50 cases in each group.The erythromycin group was given erythromycin injection,and the lavage group was given bronchofibroscope alveolar lavage combined with erythromycin injection.The time of symptom disappearance,levels of inflammatory factors,lung function indexes and clinical efficacy in the two groups were compared.Results:The time of cough cessation,time of rale disappearance and time of body temperature normalization in the lavage group were shorter than those in the erythromycin group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in erythrocyte sedimentation rate(ESR),procalcitonin(PCT)and C-reactive protein(CRP)levels between the two groups(P>0.05).After treatment,the levels of ESR,PCT and CRP in the lavage group were lower than those in the erythromycin group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in forced vital capacity(FVC)and forced expiratory volume in the first second(FEV_(1))between the two groups(P>0.05).After treatment,the levels of FVC and FEV_(1) in the lavage group were higher than those in the erythromycin group,and the differences were statistically significant(P<0.05).The total effective rate of the lavage group was higher than that of the erythromycin group,and the difference was statistically significant(P<0.05).Conclusion:The clinical effect of bronchofibroscope alveolar lavage combined with erythromycin injection in the treatment of mycoplasma pneumonia in children is significant,which can effectively shorten the duration of clinical symptoms,reduce the number of mycoplasma pneumoniae,improve the lung function and accelerate the recovery of children,and is worth promoting and applying.
