改良早期预警评分系统应用于院前急救评估的效果研究

Effect of Improved Early Warning Score System Applied in Pre-Hospital First Aid Evaluation

目的:探讨改良早期预警评分系统(NEWS)应用于院前急救评估的效果。方法:选取广州市番禺区何贤纪念医院急诊科2020年12月-2021年12月收治的100例患者,按照系统随机化法分为对照组和试验组,每组50例。对照组实施常规护理院前急救,试验组实施改良NEWS院前急救。比较两组患者的24 h留观意外事件和1个月内再次急诊发生率、去向情况以及患者满意度。结果:试验组患者的24 h留观意外事件发生率低于对照组,1个月内再次急诊率低于对照组,差异有统计学意义(P<0.05)。试验组患者的住院率低于对照组,转ICU率低于对照组,差异有统计学意义(P<0.05);两组患者的死亡率比较,差异无统计学意义(P>0.05)。试验组患者对护理制度、护理沟通、护理环境、护理服务的满意度评分及总满意度评分高于对照组,差异有统计学意义(P<0.05)。结论:改良NEWS可降低院前急救患者的24 h留观意外事件发生率、1个月内再次急诊率、住院率、转ICU率、死亡率,提高院前急救患者的满意度。

Objective:To explore the effect of improved early warning score system(NEWS)in the evaluation of pre-hospital first aid.Methods:A total of 100 patients who were received by the Emergency Department of He Xian Memorial Hospital in Panyu District of Guangzhou from December 2020 to December 2021 were selected and divided into control group and experimental group according to the systematic randomization method,with 50 cases in each group.The control group received routine nursing and pre-hospital first aid,and the experimental group received improved NEWS for pre-hospital first aid.The incidence of 24-hour observation unexpected events and re-emergency within 1 month,whereabouts and patient satisfaction were compared between the two groups.Results:The incidence of 24-hour observation unexpected events in the experimental group was lower than that in the control group,and the re-emergency rate within 1 month in the experimental group was lower than that in the control group,with statistically significant difference(P<0.05).The hospitalization rate in the experimental group was lower than that in the control group and the ICU transfer rate in the experimental group was lower than that in the control group,with statistically significant difference(P<0.05);There was no significant difference in mortality between the two groups(P>0.05).The scores of patient's satisfaction with nursing system,nursing communication,nursing environment and nursing service in the experimental group were higher than those in the control group,with statistically significant difference(P<0.05).Conclusion:The improved NEWS can reduce the incidence of 24-hour observation unexpected events,re-emergency rate within one month,hospitalization rate,ICU transfer rate and mortality of pre-hospital emergency patients,and improve the satisfaction of prehospital emergency patients.
