活血壮骨汤联合股四头肌功能锻炼治疗膝骨关节炎临床研究

Clinical Study on Huoxue Zhuanggu Tang Combined with Quadriceps Functional Exercising for Knee Osteoarthritis

目的:观察活血壮骨汤联合股四头肌功能锻炼治疗膝骨关节炎的临床疗效。方法:选择120例膝骨关节炎患者,依据随机数字表法分为观察组和对照组各60例。2组均给予双醋瑞因胶囊治疗,对照组在此基础上进行股四头肌功能锻炼,观察组在对照组基础上使用活血壮骨汤治疗。2组均治疗6周。比较2组治疗前后视觉模拟评分法(VAS)、西安大略和麦克马斯特大学关节炎指数(WOMAC)、Lequesne指数评分,以及血清超氧化物歧化酶(SOD)、丙二醛(MDA)、白细胞介素-6(IL-6)、一氧化氮(NO)水平。结果:治疗后,2组VAS、WOMAC、Lequesne指数评分均较治疗前降低(P<0.05),观察组VAS、WOMAC、Lequesne指数评分均低于对照组(P<0.05)。治疗后,2组血清SOD水平均较治疗前升高(P<0.05),观察组血清SOD水平高于对照组(P<0.05);2组血清MDA、IL-6、NO水平均较治疗前降低(P<0.05),观察组血清MDA、IL-6、NO水平均低于对照组(P<0.05)。结论:活血壮骨汤联合股四头肌功能锻炼治疗膝骨关节炎可有效缓解患者疼痛,提高膝关节功能,其作用机制与降低血清MDA、IL-6、NO水平和提高血清SOD水平有关。

Objective:To observe the clinical effect of Huoxue Zhuanggu tang combined with quadriceps functional exercising for knee osteoarthritis.Methods:A total of 120 cases of patients with knee osteoarthritis were selected and divided into the observation group and the control group according to the random number table method,with 60 cases in each group.Both groups were treated with diacerein capsules;the control group was additionally treated with quadriceps functional exercising,and the observation group was additionally treated with Huoxue Zhuanggu tang based on the treatment of the control group.Both groups were treated for six weeks.Before and after treatment,the scores of Visual Analogue Scale(VAS),Western Ontario and Mcmaster Universities Osteoarthritis Index(WOMAC)and Lequesne Index,and the levels of superoxide dismutase(SOD),malondialdehyde(MDA),interleukin-6(IL-6)and nitric oxide(NO)in serum in the two groups were compared.Results:After treatment,the scores of VAS,WOMAC and Lequesne Index in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of VAS,WOMAC and Lequesne Index in the observation group were lower than those in the control group(P<0.05).After treatment,the SOD levels in serum in the two groups were increased when compared with those before treatment(P<0.05),and the SOD level in serum in the observation group was higher than that in the control group(P<0.05);the levels of MDA,IL-6 and NO in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of MDA,IL-6 and NO in serum in the observation group were lower than those in the control group(P<0.05).Conclusion:Huoxue Zhuanggu tang combined with quadriceps functional exercising for knee osteoarthritis can effectively relieve pain and improve knee joint function,whose mechanism may be related to the decrease of levels of MDA,IL-6 and NO in serum and the increase of SOD levels in serum.
