基于现代文献分析冠心病心衰“和法”组方配伍规律

Analysis of Medication Rules of Harmonization Method in Cardiovascular disease on Literature Research

目的 系统整理并归纳总结冠心病心衰“和法”的临床组方用药规律,为中医药治疗心血管疾病提供选方用药参考。方法 在中国知网(CNKI),万方数据库(Wan Fang),维普(VIP)等数据库中检索自2000年至2021年所有关于“和法”中药复方治疗冠心病心衰的相关文献,提取方剂名称、药物组成、统计药物使用频率、药物类别,利用SPSS Modeler18.0对高频药物进行关联规则分析,并通过SPSS Statistics 26进行系统聚类分析。结果 研究最终纳入45篇合格文献,涉及43首方剂,144种药物,药物总频次为815次,其中有22味中药使用频次≥10次,使用频率较高的前3位分别为甘草(6.5%),柴胡(5.0%),茯苓(4.2%)。共涉及15种药物类别,其中补虚药(29.4%),解表药(13.5%),活血化瘀药(9.9%),使用频率最高。将使用频次≥10次的中药进行关联规则分析显示,22味中药中二项关联共12对,以甘草配伍茯苓、白芍,柴胡配伍黄芩、半夏为主;三项关联共11组,以半夏、黄芩、柴胡等组合为主。进行系统聚类分析显示,22味中药聚为6类时聚类效果最好。结论 冠心病心衰“和法”表现为:损阴和阳、损阳和阴、逐瘀和血、泻水和气、寒热并用、补泻兼施、心肝脾同调、调和血-脉-心-神,可以指导冠心病心衰的方方面面,对冠心病心衰的防治具有一定的借鉴意义。

Objective To systematically sort out and summarize the medication rules of clinical prescription and medication rules of "harmony method" for coronary heart disease with heart failure, and to provide reference for selecting prescriptions and medications for the treatment of coronary heart disease with heart failure with traditional Chinese medicine.Methods All the relevant literatures concerning "harmony method" on the treatment of coronary heart disease with heart failure with compound TCM prescriptions from 2000 to 2021 were retrieved from the CNKI, Wan Fang, VIP and other databases. and the names of prescriptions and drug components were extracted, followed by the frequency of drug use and drug category. Association rules of high-frequency drugs were analyzed by SPSS modeler 18.0,and systematic clustering analysis was conducted by SPSS 26.0.Result 45 eligible literatures were included, involving 43 prescriptions and 144 kinds of drugs. The total frequency of drugs was 815 times. Among them, 22 kinds of traditional Chinese medicine were used more than 10 times, and the top three were licorice(6.5%), bupleurum(5.0%) and tuckahoe(4.2%). A total of 15 types of drugs were involved, among which tonic drugs(29.4%), relieving drugs(13.5%) and promoting blood circulation and removing blood stasis drugs(9.9%) were the most frequently used. The results of association rule analysis showed that there were 12 pairs of binomial Association in 22 kinds of traditional Chinese medicine, mainly licorice and tuckahoe, paeony, bupleurum and Jaundice, pinellia ternate;There were 11 groups of three associations, mainly Pinellia ternata, Jaundice and bupleurum. The results of cluster analysis showed that the clustering effect was the best when 22 traditional Chinese medicines were clustered into 6 groups.Conclusion The "harmony method" of coronary heart disease with heart failure is shown as: Damaging Yin and Yang, damaging Yang and Yin, removing stasis and blood, purging water and Qi, combining cold and heat, reinforcing and purging simultaneously, harmonizing heart, liver and spleen, regulating blood-pulse-heart-spirit, which can direct all aspects of coronary heart disease with heart failure, it has certain reference significance for the prevention and treatment of coronary heart disease with heart failure.
