基于“肝主疏泄”探讨高血压病

高血压病属于中医“眩晕”“头痛”“肝风”等范畴,病位主要涉及肝脾肾三脏,其中与肝的关系最为密切。基于“肝主疏泄”理论,通过深入分析高血压病的病机演变,多为肝阳化风、肝火上炎、肝旺痰阻、肝郁血瘀、肝阴(血)亏虚证,临床分别运用平肝潜阳法、清肝泻火法、镇肝祛痰法、疏肝活血法、柔肝滋阴法治疗。高血压病的发生发展虽多及于肝,但不独责于肝,临床应用过程中应灵活运用中医整体观念为患者制定符合自身情况的个体化诊疗方案,方能取得良好疗效。

Hypertension belongs to the categories of "vertigo", "headache" and "liver wind" in traditional Chinese medicine.The location of the disease mainly involves liver, spleen and kidney, of which the relationship with the liver is mostly close.Based on the theory of "liver controlling soothing" and through in-depth analysis of the pathogenesis evolution of hypertension, most of them are liver-Yang transforming wind, liver-fire inflammation, liver hyperactivity and phlegm obstruction, liver depression and blood stasis, and liver-Yin(blood) deficiency syndrome.Clinically, they are treated with the methods of calming liver and subduing Yang, clearing liver and purging fire, calming liver and eliminating phlegm, soothing liver and activating blood circulation, softening liver and nourishing Yin.Although the occurrence and development of hypertension mostly involve liver, it is not solely responsible for the liver.In the process of clinical application, we should flexibly use the overall concept of traditional Chinese medicine to formulate individualized diagnosis and treatment plans for patients in line with their own conditions, so as to achieve good curative effects.
