谷红注射液联合丹参多酚酸盐方案在急性脑挫伤中的应用价值及作用机制评定 被引量:1

摘要 目的评定谷红注射液联合丹参多酚酸盐方案在急性脑挫伤中的应用价值及作用机制。方法研究采用一次性横断面观察方法,并根据多阶段整群随机抽样法且在保证整体抽样准确性的前提下,最终抽取2019年1月—2021年1月于医院确诊的急性脑挫伤患者60例作为研究对象。按照数表法将患者随机分为对照组(30例)和实验组(30例);对照组给予常规干预措施,实验组在对照组基础上给予谷红注射液联合丹参多酚酸盐方案,7 d为1个疗程,连续观察两个疗程。比较两组患者治疗前后中医证候评分、格拉斯哥昏迷量表评分(Glasgow)、神经功能缺损积分(NIHSS)和血液流变学指标及血清星形胶质源性蛋白(S-100β)表达、高迁移率族蛋白B1(HMGB1)含量。结果实验组中医临床显效率[53.33%(16/30)]、总有效率[96.67%(29/30)],分别高于对照组[36.67%(11/30)、80.00%(24/30)],差异有统计学意义(P<0.05)。治疗后较治疗前,两组患者Glasgow评分升高、NIHSS评分和血清S-100β表达、HMGB1含量均下降,差异有统计学意义(P<0.05);治疗后,实验组患者Glasgow评分高于对照组,NIHSS评分和血清S-100β表达、HMGB1含量均低于对照组,差异有统计学意义(P<0.05)。治疗后较治疗前,实验组患者全血高切黏度、低切黏度、血浆黏度、红细胞压积均下降,对照组仅全血低切黏度下降,差异有统计学意义(P<0.05);治疗后,实验组患者全血高切黏度、低切黏度、血浆黏度、红细胞压积均低于对照组,差异有统计学意义(P<0.05)。结论谷红注射液联合丹参多酚酸盐可使“活血化瘀、通络护脑”作用倍增,利于急性脑挫伤后脑之功能快速重建,缩短神机失用时间。 Objective To evaluate the efet and mechanism of Guhong Injection(谷红注射液)combined with salvianolate in patients with acute brain contusion.Methods A one-time cross-sectional observation method was used,and according to the multi-stage cluster random sampling method,under the premise of guaranteeing the accuracy of the whole sampling,a total of 60 patients with acute brain contusion diagnosed between January 2019 and 2021 were selected for the study.The patients were ran-domly divided into the control group(30 cases)and the experimental group(30 cases).The control group was given routine inter-vention measures,and the experimental group was given Guhong Injection combined with salvianolate on the basis of the control group.Seven days was a course.The scores of TCM syndromes,Glasgow coma scale(Glasgow),score of neurological defici(NIH-SS),expression of astrocyte-derived protein(S-100B)and high mobility group box-1(HMGBI)were compared between the two groups before and after treatment.Results The clinical efctive rate of the experimental group was 53.33%(16/30)and the total efective ralte was 96.67%(29/30),which were higher than those of the control group[(36.67%,11/30)and 80.00%(24/30)],respectively,with satitical significance(P<0.05).After treatment,the Clasgow Score,NIHSS score,serum s-100βexpression and HMGBI content in the two groups were significantly higher than those before treatment(P<0.05).Ater treatment,the Glasgow score was higher in the experimental group than that in the control group.The NIHSS score,the expression of S-100βand the content of HMGB1 in serum in the experiment group were significantly lower than those in control group(P<0.05).After treatment,the whole blood high shear viscosity,low shear viscosity,plasma viscosity and hematocrit decreased in the experimental group,but only the whole blood low shear viscosity decreased in the control group(P<0.05).The whole blood high shear viscosity,low shear viscosity,plasma viscosity and hematocrit in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion Guhong Injection combined with salvanolate can double the ffect of promoting blood cireulation and removing blood stasis,dredging ollaterals and protecting the brain.
