金水六君煎合补肺汤加减对慢性阻塞性肺病稳定期血氧指标、症状和肺呼吸功能的影响

Effects of Jinshui Liujun Decoction(金水六君煎)Combined with Modified Bufei Decoction(补肺汤)on Blood Oxygen Indexes,Symptoms and Pulmonary Respiratory Function in Patients with Stable COPD

目的研究金水六君煎合补肺汤加减对慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)稳定期患者血氧指标、症状和肺呼吸功能的影响。方法选择2018年4月—2020年5月于医院就诊的100例COPD稳定期患者,按随机数表法分为观察组50例和对照组50例。对照组接受常规治疗,观察组同时服用金水六君煎合补肺汤加减治疗。观察两组临床疗效、治疗前及治疗2个月后动脉血氧分压(PaO)、动脉血二氧化碳分压(PaCO)、血氧饱和度(SpO)等血氧指标、中医证候评分、第1秒用力呼出最大气量容积(FEV1)、单次用力肺活量(FVC)、FEV1/FVC等肺呼吸功能检测指标、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)等炎症因子水平变化及药物不良反应。结果治疗后,观察组总有效率为94.00%(47/50),高于对照组总有效率(80.00%,40/50)(P<0.05);观察组PaO、SpO水平高于对照组,PaCO水平低于对照组(P<0.05);观察组各项中医证候评分均高于对照组(P<0.05);观察组FEV1、FVC、FEV1/FVC水平均高于对照组(P<0.05);观察组TNF-α、IL-6、IL-8水平均低于对照组(P<0.05);两组治疗期间均未出现不良反应。结论金水六君煎合补肺汤加减治疗COPD稳定期患者临床疗效较好,可缓解患者临床症状表现,有助于提升患者血氧饱和度、促进肺功能恢复、缓解体内炎症,药物安全性好。

Ojective To study the ffects of Jinshui Liujun Decoction(金水六君煎)combined with Modified Bufei Decoction(补肺汤)on blood oxygen indexes,symptoms and pulmonary respiratory function in patients with stable chronic obstructive pul-monary disease(COPD).Methods A total of 100 patients with stable COPD treated in the hospital from April 2018 to May 2020 were enrolled and divided into the observation group(50 cases)and the control group(50 cases)according to random number ta-ble method.The control group underwent routine treatment,while the observation group was given Jinshui Liujun Decoction com-bined with Modified Bufei Decoction.The clinical curative ffect,blood oxygen indexes[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),blood oxygen saturation(SpO2)],scores of TCM syndromes,pulmo-nary respiratory function indexes[forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),FEV1/FVC],chan-ges in levels of inlammatory factors[tumor necrosis factor-a(TNF-a),interleukin-6(IL-6),interleukin-8(L.-8)]and adverse drug reactions before treatment and after 2 months of treatment in both groups were observed.Results After treatment,the total response rate of the observation group was higher than that of the control group[94.00%(47/50)us 80.00%(40/50)](P<0.05).The levels of PaO2 and SpO2 in the observation group were higher than those in the control group,while the PaCO2 level was lower than that in the control group(P<0.05).The scores of TCM syndromes in the observation group were higher than those in the control group(P<0.05).The levels of FEV1,FVC and FEVI/FVC were higher than those in the control group(P<0.05)and the levels of TNF-ax,IL6 and IL-8 were lower than those in the control group(P<0.05).There were no ad-verse reactions during treatment in either group.Conclusion The clinical curative effect of Jinshui Liujun Decoction combined with Modified Bufei Decoction in the treatment of patients with stable COPD is effective,which can relieve clinical symptoms,improve blood oxygen saturation,promote the recovery of pulmonary function and relieve inflammation,with good medication safety.
