四磨汤口服液对腹腔镜阑尾切除术后胃肠功能改善的作用分析

Improvement Effect of Simo Decoction Oral Liquid on Gastrointestinal Function after Laparoscopic Appendectomy

目的分析四磨汤口服液在腹腔镜阑尾切除术后对胃肠功能的改善作用。方法对68例急性阑尾炎患者进行研究,患者均于2019年11月—2021年12月就诊于南京市高淳中医院普外科,均行腹腔镜阑尾切除术,按照随机数字表法分组。对照组34例,术后予以常规干预措施,观察组34例,在对照组基础上联合使用四磨汤口服液,对比临床疗效、治疗前后疼痛程度、中医证候积分、胃肠激素、两组术后胃肠功能恢复时间。结果观察组总有效率为97.06%,高于对照组的73.53%,观察组肠鸣音恢复时间、肛门排气时间、术后首次排便时间均优于对照组,组间对比差异有统计学意义(P<0.05);两组治疗前疼痛程度、中医证候积分和胃肠激素水平差异无统计学意义(P>0.05),观察组治疗后疼痛评分、中医证候积分和促肾上腺皮质激素释放激素水平更低,胃动素、胃泌素和生长激素释放肽水平更高,与对照组各项指标对比差异有统计学意义(P<0.05)。结论在腹腔镜阑尾切除术后使用四磨汤口服液,可缓解疼痛感,改善临床症状,可对胃肠激素水平予以调节,可加快胃肠功能恢复速度,促进疾病治疗效果的提升,有推广价值。

Objective To analyze the improvement effect of Simo Decoction Oral Liquid on gastrointestinal function after laparoscopic appendectomy.Methods A total of 68 patients with acute appendicitis undergoing laparoscopic appendectomy in the Department of General Surgery of Nanjing Gaochun Traditional Chinese Medicine Hospital from November 2019 to December 2021 were selected and divided in control group and observation group according to the hospital admission order,34 in each group.Patients in the control group were given routine postoperative interventions,while those in the observation group received Simo Decoction Oral Liquid on the basis of the control group.The clinical efficacy,pain degree before and after treatment,traditional Chinese medicine(TCM)syndrome score,gastrointestinal hormone,and postoperative recovery time of gastrointestinal function were compared between the two groups.Results The total effective rate of the observation group was 97.06%,which was higher than that of the control group.The recovery time of bowel sound,time of anal exhaust and time of first postoperative defecation were(9.18±1.21)h,(22.12±4.12)h and(40.18±4.56)h,respectively,and there were differences among the two groups(P<0.05).The pain degree,TCM syndrome score and gastrointestinal hormone level in the two groups before treatment had no difference(P>0.05).After treatment,the pain score,TCM syndrome score and the level of corticotropin releasing hormone in the observation group were lower and the levels of motilin,gastrin and growth hormone releasing peptide were higher than those in the control group(P<0.05).Conclusion The use of Simo Decoction Oral Liquid after laparoscopic appendectomy could relieve pain,improve clinical symptoms,regulate gastrointestinal hormone level,speed up the recovery of gastrointestinal function,and promote the improvement of curative effect,which was worthy of promotion.
