IgA肾病的中医诊治研究

TCM Diagnosis and Treatment of IgA Nephropathy

IgA肾病是一种常见的慢性肾小球肾炎,大约每10年有20%的患者进展至慢性肾衰竭。目前西医治疗IgA肾病主要是通过免疫抑制剂、激素及对症治疗,副作用较大且疗效不理想,而中医药治疗对IgA肾病的血尿、蛋白尿、水肿等症状具有较好的疗效且无相应副作用。该文通过查阅万方、中国知网(CNKI)、维普、PubMed等数据库自2005—2021年期间的相关文献,从病因病机、脏腑辨证(从肾论治、从肺论治、从脾论治、从肝论治)、六经辨证、三焦辨证、分型论治、分期论治、专方及单味中药治疗等几方面进行分析和总结,并提出对IgA肾病辨证治疗的拙见,旨为中医药诊治IgA肾病的进一步研究提供新线索。

IgA nephropathy is a common form of chronic glomerulonephritis that progresses to chronic renal failure in approximately 20%of patients every 10 years.At present,western medicine treats IgA nephropathy mainly through immunosuppressants,hormones,and symptomatic treatment,which has large side effects and unsatisfactory effects,while traditional Chinese medicine(TCM)has good curative effects on the symptoms of IgA nephropathy such as hematuria,proteinuria,and edema,with no corresponding side effects.By retrieving the relevant literature from Wanfang,China National Knowledge Infrastructure(CNKI),VIP Chinese Technology Periodical Database,Pub Med,and other databases from 2005 to 2021,this paper analyzed IgA nephropathy and proposed syndrome differentiation and treatment of IgA nephropathy based on the etiology and pathogenesis,visceral syndrome pattern differentiation(treatment from the kidney,treatment from the lung,treatment from the spleen,and treatment from the liver),six-meridian syndrome differentiation,triple-energizer syndrome pattern identification,treatment by type,treatment by stage,special prescription treatment,and monomer Chinese medicine treatment.This paper is expected to provide new clues for further research on TCM diagnosis and treatment of IgA nephropathy.
