1410例原发性三叉神经痛的病例系列分析

A case series analysis on 1410 primary trigeminal neuralgia patients

目的:收集就诊原发性三叉神经痛患者的病历资料,进行病例系列分析。方法:收集2012年5月1日-2021年4月30日就诊于我院诊断为原发性三叉神经痛患者的病历信息资料,分析患者的基本情况、现病史情况、既往史情况并进行数据分析。结果:共收集分析1410例患者的病历信息。显示:女性患者(66.24%)占比明显多于男性患者(33.76%),60~69岁女性患者患病人数占比最多(21.13%)。右侧发病患者占比(54.75%)明显较高。第Ⅱ支病变(40.32%)占比稍高于第Ⅲ支(38.12%),72.66%的患者疼痛表现在多个部位。绝大多数患者为多种疼痛性质共同伴随(占比92.50%)。疼痛发作频率中每日发作10次以下的患者人数占比最多(48.58%)。中度疼痛程度患者人数占比最多(61.70%)。疼痛诱发因素中饮食不节最常见(占比59.57%)。患者合并全身系统性疾病中最常见的是高血压病(占比20.07%)。63.97%的患者既往有口服药物治疗史,其中口服卡马西平等抗癫痫类药物占比最多(56.38%),口服药物不良反应中最常见的是头晕(占比32.06%)。9.79%的患者既往有外科手术治疗史。结论:分析总结原发性三叉神经痛的发病特征,为临床快速准确诊断、及时有效治疗及后期疗效评价提供依据。

Objective:To collect medical records of patients with primary trigeminal neuralgia and make a case series analysis.Methods:The medical records of patients diagnosed with primary trigeminal neuralgia in our hospital from May 1,2012 to April 30,2021 were collected.The basic information,present medical history and past history of the patients were analyzed and the data were analyzed.Results:The medical records of 1410 patients were collected and analyzed.The results showed that the proportion of female patients(66.24%)was significantly higher than that of male patients(33.76%),and the proportion of female patients aged 60-69 years was the largest(21.13%).The proportion of patients with right-sided disease was significantly higher(54.75%).The proportion of lesions in the second branch(40.32%)was slightly higher than that in the third branch(38.12%),and 72.66%of patients had pain in multiple sites.The majority of patients had multiple pain properties(92.50%).Patients with less than 10 pain attacks per day accounted for the largest proportion(48.58%).The proportion of patients with moderate pain was the largest(61.70%).Improper diet was the most common cause of pain(59.57%).Hypertension was the most common systemic disease(20.07%).63.97%of the patients had a history of oral drug therapy,among which oral carbamazepine antiepileptic drugs accounted for the most(56.38%),and dizziness was the most common adverse reaction of oral drugs(32.06%).9.79%patients had a history of surgical treatment.Conclusion:By analyzing and summarizing the characteristics of primary trigeminal neuralgia,we can provide the basis for clinical accurate diagnosis,treatment and later curative effect evaluation.
