DTI对颅内良、恶性肿瘤鉴别诊断及在临床手术方案制定中的临床价值研究

摘要 目的分析了扩散张量成像(DTI)对颅内良、恶性肿瘤鉴别诊断及在临床手术方案制定中的临床价值。方法选取53例颅内肿瘤患者作为研究对象(研究组)。同期选取34例颅内占位患者作为对照组,比较颅内良、恶性肿瘤的FA值、DCavg值及良、恶性肿瘤脑白质纤维束改变情况;分析两组患者肿瘤切除情况,并比较两组致残率。结果良性组肿瘤实质区DCavg值、瘤周水肿区FA值均明显高于恶性组(P<0.05);良性肿瘤以移位为主,浸润性改变较少见,未见纤维束中断;恶性肿瘤瘤周纤维束破坏较良性肿瘤多见(P<0.05)。对照组全切率、致残率分别为50.00%、35.29%;研究组全切率、致残率分别为83.02%、7.55%,研究组全切率显著高于对照组,致残率显著低于对照组(P<0.05)。结论DTI多参数值FA值、DCavg值在鉴别诊断颅内良、恶性肿瘤中具有重要价值,可为临床手术方案制定提供可靠的依据,提高肿瘤全切率,降低致残率。 Objective To analyze the clinical value of diffusion tensor imaging(DTI)in the differential diagnosis of intracranial benign and malignant tumors and in the formulation of clinical surgical plans.Method 53 patients with intracranial tumors were selected as the research object(research group).34 patients with intracranial space-occupying at the same time were selected as the control group.The FA values,DCavg values and the changes in fiber bundles of white matter in benign and malignant tumors were compared.The tumor resections of patients between the two groups were analyzed,and the disability rates between the two groups were compared.Results The DCavg value in the parenchymal area of tumor and the FA value in the peritumoral edema area of the benign group were significantly higher than those of the malignant group(P<0.05).The benign tumors were mainly shifted,and the infiltrating changes were rare,and there was no interruption of the fiber bundles.The destruction of peritumoral fiber bundles in malignant tumors was more common than in benign tumors(P<0.05).The rate of complete resection and disability rate in the control group were 50.00%and 35.29%respectively,and the research group were 83.02%and 7.55%,respectively.The rate of complete resection in the study group was significantly higher than that in the control group,and the disability rate was significantly lower than that in the control group(P<0.05).Conclusion The DTI multi-parameter values FA value and DCavg value are of great value in the differential diagnosis of intracranial benign and malignant tumors,which can provide a reliable basis for the formulation of clinical surgical plans,increase the rate of complete resection and reduce the disability rate.
