MRI-DWI及CT在直肠癌术前诊断中的应用分析

Application of MRI-DWI and CT in Preoperative Diagnosis of Rectal Cancer

目的探讨磁共振扩散加权成像(MRI-DWI)及CT成像在直肠癌术前诊断中的应用价值。方法回顾性选择我院2017年4月至2021年4月接受的83例直肠癌患者,均经病理解剖证实并符合直肠癌,所有患者术前均行MRI-DWI及CT成像检查,根据扫描图像进行术前分期评估,并以组织病理学T分期为“金标准”,比较两种检查方式在术前诊断中的应用效果。结果83例直肠癌患者中,病理诊断结果显示T1期5例、T2期21例、T3期43例、T4期14例;MRI-DWI检查显示T1期5例、T2期19例、T3期40例、T4期13例,诊断符合率分别为100%、90.18%、93.02%、92.86%;CT成像结果显示T1期4例、T2期17例、T3期33例、T4期12例,诊断符合率分别为80.00%、80.95%、76.74%、85.71%;MRI-DWI检查总符合率比CT成像高,差异具有统计学意义(P<0.05)。结论MRI-DWI及CT在直肠癌术前诊断中均有较好的诊断效果,但前者在T3分期评估中的准确率更高,具有较高的应用价值,因此可将MRI-DWI作为直肠癌术前诊断及分期的首选检查方法之一。

Objective This study aims to discuss the application value of magnetic resonance imaging diffusion weighted imaging(MRI-DWI)and CT in preoperative diagnosis of rectal cancer.Methods Totally 83 patients with rectal cancer confirmed by pathological anatomy in the hospital were retrospectively selected between April 2017 and April 2021.All were subjected to MRI-DWI and CT before surgery.Preoperative staging was performed based on the images obtained.Histopathological T staging was taken as the golden standard to compare the application effects of the two examination methods in preoperative diagnosis.Results Pathological results showed 5 cases in T1 stage,21 cases in T2 stage,43 cases in T3 stage,and 14 cases in T4 stage.MRI-DWI showed 5 cases in T1 stage,19 cases in T2 stage,40 cases in T3 stage,and 13 cases in T4 stage,with coincidence rates of 100%,90.18%,93.02%,and 92.86%.CT showed 4 cases in T1 stage,17 cases in T2 stage,33 cases in T3 stage,and 12 cases in T4 stage,with coincidence rates of 80.00%,80.95%,76.74%,and 85.71%.The overall coincidence rate of MRI-DWI was higher than that of CT(P<0.05).Conclusion Both MRI-DWI and CT are effective in preoperative diagnosis of rectal cancer,but the former is more accurate in the evaluation of T3 staging and has higher application value.Therefore,MRI-DWI can be used as the preferred methodfor preoperative diagnosis and staging of rectal cancer.
