经颅多普勒超声发泡试验联合BE-FAST量表在隐源性脑卒中的应用价值

Application Value of Transcranial Doppler Ultrasound Foaming Test Combined with BE-FAST Scale in Patients with Cryptogenic Stroke

目的:探究经颅多普勒超声发泡试验(c-TCD)联合BE-FAST量表在隐源性脑卒中的应用价值.方法:选择2019年8月至2021年6月本院收治的疑似隐源性脑卒中患者60例为研究对象,所有患者均接受c-TCD与BE-FAST量表评估.分析c-TCD及c-TCD联合BE-FAST量表在隐源性脑卒中的诊断价值,并计算与隐源性脑卒中诊断"金标准"的一致性.结果:c-TCD联合BE-FAST量表在隐源性脑卒中诊断中敏感度、准确度及阴性预测值均高于单独c-TCD检查(P<0.05),kappa检验显示:c-TCD检查与隐源性脑卒中诊断"金标准"的一致性尚可(kappa=0.555,P<0.001);c-TCD联合BE-FAST量表与隐源性脑卒中诊断"金标准"的一致性良好(kappa=0.893,P<0.001).结论:c-TCD联合BE-FAST量表在隐源性脑卒中患者诊断中能够及时察觉患者病情变化,从而对病因进行诊断,有助于提高诊断效能.

Objective:To explore the application value of transcranial Doppler ultrasound foaming test(c-.TCD)combined with BE-FAST scale in patients with cryptogenic stroke.Methods:Sixty patients with suspected cryptogenic stroke admitted to our hospital from August 2019 to June 2021 were selected as research swbjects.All patients were evaluated by c-TCD and BE-FAST scales.The diagnostic value of c-TCD and c-TCD combined with BE-FAST scale in implicit stroke was analyzed,and its consistency with the diagnostic"gold standard"of cryptogenic stoke was calculated.Results:The sensitivity,accuracy and negative predictive value of c-TCD combined with BE-FAST in the diagnosis of cryptogenic stroke were higher than those of c-TCD only(P<0.05).Kappa test showed that:the consistency between c-TCD and diagnostic"gold standard"of cryptogenic stoke was acceptable(Kappa=0.555,P<0.001).The c-TCD combined with BE-FAST scale was in good agreement with diagnostic"gold standard"of cryptogenic stoke(Kappa=0.893,P<0.001).Conclusion:C-TCD combined with BE-FAST scale can detect the change of patients'condition in time in the diagnosis of cryptogenic stroke,so as to diagnose the cause of the disease,which is helpful to improve the diagnostic efficiency.
