辛菀止咳散联合穴位敷贴治疗儿童上气道咳嗽综合征痰湿恋肺证的临床研究

Efficacy of Xinyuan Zhike powder combined with acupoint application on children upper airwaycough syndrome with syndrome of phlegm-damp lingering the lung

目的评价辛菀止咳散联合穴位敷贴治疗上气道咳嗽综合征痰湿恋肺证患儿的临床疗效。方法招募2019年12月—2021年6月在上海中医药大学附属龙华医院儿科门诊就诊的上气道咳嗽综合征痰湿恋肺证患儿120采用随机数字表法分为对照组及观察组,每组60例。对照组给予辛菀止咳散口服,观察组在口服辛菀止咳散的基础上给予穴位贴敷,2组均治疗14 d。观察2组治疗前后咳嗽积分、临床症状(流涕、喷嚏、咽部充血、咽痒、鼻塞、鼻痒、咳痰、清嗓)积分、中医症状积分变化,比较2组治疗效果及不良反应发生情况。结果2组治疗后咳嗽积分、各项临床症状积分、中医症状积分均显著低于治疗前(P均<0.05),且观察组咳嗽积分、中医症状积分和临床症状中流涕、喷嚏、咽痒、鼻塞、鼻痒、清嗓积分均明显低于对照组(P均<0.05)。治疗2周后,观察组愈显率为93.3%(56/60),对照组愈显率为33.3%(20/60),观察组愈显率明显高于对照组(P<0.05)。2组患儿治疗过程中均未出现不良反应。结论辛菀止咳散单用及联合穴位敷贴治疗小儿上气道咳嗽综合征均有较好的效果,联合治疗疗效更佳,症状改善更明显。

Objective It is to evaluate the clinical efficacy of Xinyuan Zhike powder combined with acupoint application on children upper airway cough syndrome with syndrome of phlegm-damp lingering the lung.Methods A total of 120 children with upper airway cough syndrome with syndrome of phlegm-damp lingering the lung treated in Department of Pediatrics,Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from December 2019 to June 2021 were recruited and randomly divided into observation group and control group according to random number method,with 60 cases in each group.The control group was treated with Xinyuan Zhike powder orally,and the observation group was given acupoint application on the basis of oral Chinese medicine.The treatment course for the two groups lasted for 2 weeks.The changes in the scores of cough,scores of clinical symptoms(runny nose,sneezing,pharyngeal congestion,pharyngeal itching,nasal congestion,nasal itching,expectoration,throat clearing),and scores of traditional Chinese medicine syndrome were observed before and after treatment in the two groups,the treatment effect and occurrence of side effect were compared between the two groups.Results After treatment,the scores of cough,scores of every clinical symptom,and scores of traditional Chinese medicine syndrome in both groups were significantly lower than those before treatment(all P<0.05),and the scores of cough,scores of TCM symptoms and scores of clinical symptoms such as runny nose,sneezing,pharyngeal itching,nasal congestion,nasal itching,throat clearing in the observation group were significantly lower than those in the control group(all P<0.05).After 2 weeks of treatment,the effective rate was 93.3%(56/60)in the observation group and 33.3%(20/60)in the control group,the effective rate in the observation group was significantly higher than that in the control group(P<0.05).There were no adverse reactions of all patients during the treatment.Conclusion Both Xinyuan Zhike powder alone and Xinyuan Zhike powder combined with acupoint application and Chinese medicine combined with acupoint application have good therapeutic effects on children with upper airway cough syndrome,but the Chinese medicine combined with acupoint application is more effective and can improve clinical symptoms more significantly.
