以手功能康复理念为指导的手法淋巴引流对患者乳腺癌术后淋巴水肿的影响

Effect of Manual Lymphatic Drainage Guided by the Concept of Hand Function Rehabilitation on Postoperative Lymphedema After Breast Cancer Surgery

目的探讨以手功能康复理念为指导的手法淋巴引流对患者乳腺癌术后淋巴水肿的影响。方法选取2020年2—12月于河南科技大学附属三门峡市中心医院接受乳腺癌改良根治术的42例乳腺癌患者作为对照组,术后接受常规护理干预,选取2021年1—11月于医院接受乳腺癌改良根治术的42例乳腺癌患者作为观察组,术后在对照组基础上接受以手功能康复理念为指导的手法淋巴引流干预。比较两组患者干预后淋巴水肿发生情况、上肢功能情况、心理状态及生活质量水平。结果干预后观察组淋巴水肿总发生率低于对照组(P<0.05);干预后两组患者上肢功能评分均降低,观察组评分低于对照组(P<0.05);干预后两组患者焦虑自评量表(SAS)与抑郁自评量表(SDS)评分均降低,且观察组评分低于对照组(P<0.05);干预后观察组生活质量各维度评分高于对照组(P<0.05)。结论以手功能康复理念为指导的手法淋巴引流能降低乳腺癌患者术后淋巴水肿发生率,缓解不良情绪,提升上肢活动度功能与生活质量。

Objective To investigate the effect of manual lymphatic drainage guided by the concept of hand function rehabilitation on postoperative lymphedema in patients with breast cancer.Methods A total of 42 patients with breast cancer who received modified radical mastectomy in Sanmenxia Central Hospital Affiliated to Henan University of Science and Technology from February to December 2020 were selected as the control group,and routine nursing intervention was performed after operation.A total of 42 patients with breast cancer who received modified radical mastectomy in hospital from January to November 2021 were selected as the observation group,and manual lymphatic drainage intervention guided by the concept of hand function rehabilitation was given after operation on the basis of the control group.The incidence of lymphedema,upper limb function,psychological status and quality of life after intervention were compared between the two groups.Results After intervention,the total incidence of lymphedema in the observation group was lower than that in the control group(P<0.05).After intervention,the scores of upper limb function in both groups were lower,and the scores in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in the two groups decreased,and the scores in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of quality of life of all dimensions in the observation group were higher than those in the control group(P<0.05).Conclusion Guided by the concept of hand function rehabilitation,manual lymphatic drainage can reduce the incidence of postoperative lymphedema in breast cancer patients,alleviate bad emotions,and improve the function of upper limb mobility and quality of life.
