健脾解毒化瘀汤联合射频消融治疗对脾虚毒瘀互结型小肝癌患者甲胎蛋白及免疫功能的影响

Effect of Jianpi Jiedu Huayu Decoction Combined with Radiofrequency Ablation on alpha-Fetoprotein and Immune Function of Small Liver Cancerpatients with Spleen Deficiency Combined with Poison and Stasis

[目的]探讨健脾解毒化瘀汤联合射频消融(RFA)治疗对脾虚毒瘀互结型小肝癌患者甲胎蛋白水平、免疫功能及中医症状评分的影响。[方法]将100例脾虚毒瘀互结型原发性小肝癌患者分为对照组和治疗组各50例。对照组给予单纯RFA治疗,治疗组在行RFA治疗后第1天开始服用健脾解毒化瘀汤,每日1剂,连服30 d。[结果]两组RFA治疗后第7 d、第30 d甲胎蛋白及中医症状评分均显著降低,且治疗组降低更为明显,差异均具有统计学意义(P<0.05)。两组RFA治疗后第7 d胰岛素样生长因子-Ⅱ(IGF-Ⅱ)、转化生长因子-β_(1)(TGF-β_(1))、白介素-10(IL-10)水平较入院第1 d均明显降低,且治疗组降低更为显著,差异均具有统计学意义(P<0.05);两组RFA治疗后第30 d IGF-Ⅱ、TGF-β_(1)、IL-10水平较入院第1 d、RFA治疗后第7 d均明显降低,且治疗组降低更为显著,差异均有统计学意义(P<0.05)。[结论]健脾解毒化瘀汤联合RFA治疗脾虚毒瘀互结型原发性小肝癌临床疗效好,有助于提高患者免疫功能,值得临床推广应用。

[Objective]To investigate the effect of Jianpi Jiedu Huayu decoction combined with radiofrequency ab⁃lation(RFA)on alpha-fetoprotein level and immune function in patients with small liver cancer of spleen deficiency and toxin stasis type[.Methods]A total of 100 patients with primary small liver cancer were divided into control group and treatment group,with 50 cases in each group.The control group was given RFA alone,while the treatment group was given Jianpi Jiedu Huayu decoction since the first day after RFA treatment,once a day for 30 days[.Results]The levels of alpha-fetoprotein and TCM symptom scores on the 7th and 30th day after RFA in the treatment group and the control group were both significantly decreased,and the differences were statistically significant in the treatment group(P<0.05).The levels of IGF-Ⅱ,TGF-β_(1) and IL-10 in the treatment group on the 7th day after RFA were significantly lower than those in the control group on the 1st day after RFA,and the treatment group decreased more significantly,the differences were statistically significant(P<0.05).The levels of IGF-Ⅱ,TGF-β_(1) and IL-10 on the 30th day after RFA in the treatment group and the control group were both significantly lower than those on the 1st day after admis⁃sion and the 7th day after RFA,and the treatment group decreased more significantly,the differences were statistically significant(P<0.05).[Conclusion]Jianpi Jiedu Huayu decoction combined with RFA has a good clinical effect in the treatment of primary small liver cancer with spleen deficiency and toxin stasis which is approved helpful to improve the immune function of patients,and is worthy of clinical promotion and application.
