扎鲁司特联合沙美特罗替卡松吸入粉雾剂治疗儿童支气管哮喘的效果及对免疫功能、氧化应激水平的影响 被引量:2

摘要 目的探讨扎鲁司特联合沙美特罗替卡松吸入粉雾剂治疗儿童支气管哮喘的效果及对免疫功能、氧化应激水平的影响。方法选取我院收治的82例支气管哮喘患儿为研究对象,按照随机数字表法将其分为对照组(41例,沙美特罗替卡松吸入粉雾剂)与观察组(41例,扎鲁司特^(+)沙美特罗替卡松吸入粉雾剂)。比较两组的治疗效果。结果观察组的治疗总有效率明显高于对照组(P<0.05)。治疗后,两组的第1秒用力呼气容积(FEV1)、呼气峰流速(PEF)、第1秒用力呼气容积占用力肺活量百分比(FEV1/FVC)均升高,且观察组明显高于对照组(P<0.05)。治疗后,两组的肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)、白细胞介素-8(IL-8)水平均降低,且观察组明显低于对照组(P<0.05)。治疗后,两组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(^(+))/CD8^(+))均升高,且观察组明显高于对照组(P<0.05)。治疗后,两组的超氧化物歧化酶(SOD)水平均升高,丙二醛(MDA)水平均降低,且观察组明显优于对照组(P<0.05)。治疗后,两组的圣乔治呼吸问卷(SGRQ)评分均降低,哮喘控制测试(ACT)问卷评分均升高,且观察组明显优于对照组(P<0.05)。结论扎鲁司特联合沙美特罗替卡松吸入粉雾剂治疗儿童支气管哮喘的效果显著,可改善患儿的肺功能,减轻机体的炎症反应,提高免疫功能,进而改善患儿的氧化应激水平和哮喘症状,值得临床推广与应用。 Objective To investigate the effect of zafirlukast combined with salmeterol xinafoate and fluticasone propionate powder for inhalation on children with bronchial asthma and its influences on immune function and oxidative stress level.Methods A total of 82 children with bronchial asthma admitted in our hospital were selected as the research objects.According to the random number table method,the children were divided into control group(41 cases,salmeterol xinafoate and fluticasone propionate powder for inhalation)and observation group(41 cases,zafirlukast^(+)salmeterol xinafoate and fluticasone propionate powder for inhalation).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),and the forced expiratory volume in one second/forced vital capacity(FEV1/FVC)in both groups increased,and those in the observation group were significantly higher than the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-4(IL-4)and interleukin-8(IL-8)in both groups decreased,and those in the observation group were significantly lower than the control group(P<0.05).After treatment,the cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in both groups increased,and those in the observation group were significantly higher than the control group(P<0.05).After treatment,the level of superoxide dismutase(SOD)in both groups increased,and the level of malondialdehyde(MDA)decreased,and those in the observation group were significantly better than the control group(P<0.05).After treatment,the St.George's Respiratory Questionnaire(SGRQ)score in both groups decreased,and the Asthma Control Test(ACT)score increased,and those in the observation group were significantly better than the control group(P<0.05).Conclusion Zafirlukast combined with salmeterol xinafoate and fluticasone propionate powder for inhalation on children with bronchial asthma has a significant effect,it can improve the lung function of children,reduce the inflammatory reaction of the body,improve the immune function,and then improve the level of oxidative stress and asthma symptoms of children,which is worthy of clinical promotion and application.
