清热化瘀活血方联合西药对AECOPD患者肺功能及临床症状的影响

Effects of Qingre Huayu Huoxue decoction combined with western medicine on pulmonary function and clinical symptoms in patients with AECOPD

目的探讨清热化瘀活血方联合西药对慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能及临床症状的影响。方法选取2020年1月至12月我院收治的110例AECOPD患者作为研究对象,根据治疗方案不同将其分为对照组和观察组,各55例。对照组给予常规西药治疗,观察组给予清热化瘀活血方联合西药治疗。比较两组的中医证候积分、肺功能指标、炎症因子水平及免疫功能指标。结果治疗后,观察组的主症、次症积分低于对照组(P<0.05)。治疗后,观察组的第1秒用力呼气容积(FEV_(1))、第1秒用力呼吸容积/用力肺活量(FEV_(1)/FVC)高于对照组(P<0.05)。治疗后,观察组的降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平低于对照组(P<0.05)。治疗后,观察组的白细胞分化抗原4阳性(CD4+)及免疫球蛋白G(IgG)水平高于对照组,白细胞分化抗原8阳性(CD8+)低于对照组(P<0.05)。结论清热化瘀活血方联合西药治疗AECOPD患者可明显改善中医证候、肺功能及免疫功能。

Objective To explore the the effects of Qingre Huayu Huoxue decoction combined with western medicine on pulmonary function and clinical symptoms in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 110 patients with AECOPD admitted in our hospital from January to December 2020 were selected as the research objects,and the patients were divided into control group and observation group according to the different treatment schemes,with 55 cases in each group.The control group was treated with western medicine,and the observation group was treated with Qingre Huayu Huoxue decoction combined with western medicine.The chinese medicine syndrome scores,pulmonary function indexes,inflammatory factors levels and immune function indexes were compared between the two groups.Results After treatment,the scores of primary and secondary symptoms in the observation group were lower than those in the control group(P<0.05).After treatment,the forced expiratory volume in one second(FEV_(1))and forced expiratory volume in one second/forced vital capacity(FEV_(1)/FVC)in the observation group were higher than those in the control group(P<0.05).After treatment,the procalcitonin(PCT),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the observation group were lower than those in the control group(P<0.05).After treatment,the cluster of differentiation 4 positive(CD4+)and immunoglobulin G(IgG)in the observation group were higher than those in the control group,the cluster of differentiation 8 positive(CD8+)was lower than that in the control group(P<0.05).Conclusion The Qingre Huayu Huoxue decoction combined with western medicine on patients with AECOPD can significantly improve traditional chinese medicine syndrome,pulmonary function and immune function.
