通天口服液联合阿加曲班治疗急性脑梗死的疗效分析及机制研究

Effects analysis and mechanism of Tongtian oral liquid combined with argatroban in treating acute cerebral infarction

目的分析通天口服液联合阿加曲班治疗急性脑梗死的疗效,并探讨其治疗机制和安全性。方法选取2020年1月至2021年12月该院脑病科收治的急性脑梗死患者80例。随机分为治疗组和对照组,每组40例。对照组使用基础治疗(给予常规剂量阿司匹林、氯吡格雷及阿托伐他汀)和阿加曲班注射液治疗,治疗组在对照组治疗方案基础上加用通天口服液。观察两组疗效和安全性。结果观察组患者全血高切、低切黏度,红细胞比容,经颅多普勒-大脑中动脉阻力指数,美国国立卫生研究院卒中量表评分均明显低于对照组,BI生活指数评分明显高于对照组,差异均有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论使用通天口服液联合阿加曲班治疗急性期脑梗死可降低血液黏度,改善脑血流量,降低血管阻力,明显提升临床疗效,不良反应少,具有很高的临床使用价值。

Objective To analyze the efficacy of Tongtian oral liquid combined with argatroban in the treatment of acute cerebral infarction,and to explore its treatment mechanism and safety.Methods Eighty patients with acute cerebral infarction admitted and treated in the encephalology department of this hospital from January 2020 to December 2021 were selected and randomly divided into the control group and treatment group,40 cases in each group.The control group was treated with the basic treatment(conventional dose of aspirin,clopidogrel and atorvastatin)and argatroban injection.The treatment group was added with Tongtian oral liquid on the basis of the control group.The therapeutic efficacy and safety of the two groups were observed.Results The whole blood high shear and low shear viscosity,hematocrit,middle cerebral artery resistance index(RI)and neurological deficit score in the observation group were significantly lower than those in the control group,the BI life index score was significantly higher than that in the control group,and the differences were statistically significant(P<0.05);the occurrence rate of adverse reactions had no statistically significant difference between the two groups(P>0.05).Conclusion Tongtian oral liquid combined with argatroban in the treatment of acute cerebral infarction could reduce the blood viscosity,improve the cerebral blood flow,reduce the vascular resistance,and significantly improve the clinical efficacy with few side effects and high clinical use value.
