基于R语言分析某院处方点评大数据

Big Data Analysis of Prescription Comment in a Hospital Based on R Language

目的预警临床不合理使用频数出现较高的药品,促进合理用药。方法从某院2020年1月至12月门急诊41万张处方中按每月5000张抽取点评处方6万张,以平均用药品种数、基本药物使用率、抗菌药物使用率、注射剂使用率、医师抽样率、总体合格率为指标评价整体合理用药情况;采用4.1.0版R语言词云软件包统计“用药与临床诊断不相符”药品的出现频数,绘制不合理用药词云。结果6万张点评处方中,平均用药品种数,基本药物使用率、抗菌药物使用率、注射剂使用率,医师抽样率及总体合格率分别为(2.05±0.04)种及66.66%,14.37%,16.21%,34.58%,95.56%;不合理用药词云分析显示,阿托伐他汀钙片、兰索拉唑肠溶片、蒲地蓝消炎口服液、枸橼酸莫沙必利片出现频数最多。结论将R语言词云软件包应用于处方点评大数据分析,能直观、清晰地预警临床不合理使用的药品。

ObjectiveTo warn of the high frequency of irrational drug use in the clinic,and to promote the rational drug use.MethodsA total of 410000 outpatient and emergency prescriptions from January to December 2020 in a hospital were collected,of which 60000 prescriptions were commented,with 5000 prescriptions every month.The overall rational drug use was evaluated with the average number of drug varieties,the use rate of essential drugs,the use rate of antibacterial drugs,the use rate of injections,the sampling rate of physicians and the overall qualification rate of prescriptions as the evaluation indicators.The wordcloud software package of R language(Version 4.1.0)was used to count the frequency of drugs″inconsistent with clinical diagnosis in the drug use″,and draw the wordcloud of irrational drug use.ResultsAmong the 60000 commented prescriptions,the average number of drug varieties was(2.05±0.04)kinds,and the use rate of essential drugs,the use rate of antibacterial drugs,the use rate of injections,the sampling rate of physicians and the overall qualified rate of prescriptions were 66.66%,14.37%,16.21%,34.58%and 95.56%,respectively.The results of the wordcloud analysis of irrational drug use showed that Atorvastatin Calcium Tablets,Lansoprazole Enteric-Coated Tablets,Pudilan Xiaoyan Oral Liquid and Mosapride Citrate Tablets had the higher frequency.ConclusionThe application of wordcloud software package of R language in the big data analysis of prescription comment can realize the intuitive and clear warning of the irrational drug use in the clinic.
