椎动脉颅内段狭窄对椎动脉颅外段血流动力学的影响

Effects of intracranial stenosis of vertebral artery on hemodynamics of extracranial segment of vertebral artery

目的:研究椎动脉颅内段狭窄患者椎动脉颅外段血流动力学的变化。方法:选取2018年1月—2022年1月在泰安市中医医院行常规脑卒中超声检查,经DSA诊断椎动脉颅内段狭窄的患者70例作为研究对象,设为观察组。选择同时间段70例椎动脉无异常病变者为对照组。回顾分析观察组椎动脉病变侧与健侧颅外段椎动脉超声检查参数;比较观察组病变侧与对照组椎动脉颅外段超声检查参数。结果:观察组病变侧的颅外段椎动脉直径小于观察组健侧及对照组,椎动脉收缩期峰值流速(PSV)、舒张期末流速(EDV)低于观察组健侧及对照组,颅外段椎动脉阻力指数大于观察组健侧及对照组,差异均有统计学意义(P<0.05)。结论:椎动脉颅内段狭窄将影响椎动脉颅外段的直径,椎动脉颅外段的PSV、EDV、RI亦均发生相应变化。

Objective To study the hemodynamic changes of extracranial segment of vertebral artery in patients with intracranial segment stenosis of vertebral artery.Methods 70 patients with intracranial stenosis of vertebral artery diagnosed by DSA in Taian Traditional Chinese Medicine Hospital from January 2018 to January 2022 were selected as the observation group.Another 70 cases without abnormal lesions of vertebral artery in the same period were selected as the control group.The ultrasonic examination parameters of extracranial vertebral artery between the side of diseased and healthy side of vertebral artery in the observation group were analysed;The parameters of extracranial vertebral artery ultrasonography between the observation group and the control group were also analysed.Results The diameter of the extracranial vertebral artery on the lesion side of the observation group was smaller than that of the healthy side of the observation group and the control group,the peak systolic velocity(PSV)and end-diastolic velocity(EDV)were lower than those of the healthy side of the observation group and the control group,the arterial resistance index was greater than that of the observation group and the control group,and the difference was statistically significant(P<0.05).Conclusion Intracranial stenosis of vertebral artery will affect the diameter of extracranial segment of vertebral artery,and PSV,EDV and RI of extracranial segment of vertebral artery will change accordingly.
