复方丙酸氯倍他索软膏联合窄谱紫外线治疗轻中度银屑病的临床疗效

Clinical efficacy of compound clobetasol propionate ointment combined with narrow spectrum ultraviolet in the treatment of mild to moderate psoriasis

目的观察复方丙酸氯倍他索软膏联合窄谱紫外线治疗轻中度银屑病的临床疗效。方法选取泰州市第二人民医院2020年5月—2021年4月收治的轻中度银屑病患者94例,按照随机摸球法分为对照组与观察组,各47例。对照组予以复方丙酸氯倍他索软膏,观察组在对照组基础上联合窄谱紫外线治疗,2组均治疗8周。比较2组临床疗效,治疗前及治疗8周后临床症状评分(斑块肥厚评分、红斑症状评分、鳞屑症状评分、皮损范围评分)、银屑病面积和严重程度指数(PASI)评分、血清转化生长因子β_(1)(TGF-β_(1))水平,不良反应发生率和复发率。结果观察组治疗总有效率为97.87%,高于对照组的82.98%(χ^(2)=4.423,P=0.035)。治疗8周后,2组斑块肥厚、红斑症状、鳞屑症状、皮损范围评分低于治疗前,且观察组低于对照组(P<0.01)。治疗8周后,2组银屑病面积评分和严重程度评分低于治疗前,且观察组低于对照组(P<0.01)。治疗8周后,2组血清TGF-β_(1)水平低于治疗前,且观察组低于对照组(P<001)。2组不良反应总发生率、复发率比较,差异无统计学意义(P>0.05)。结论复方丙酸氯倍他索软膏联合窄谱紫外线治疗轻中度银屑病的临床疗效确切,可有效改善患者的临床症状,控制皮损程度,降低疾病严重性,且安全性较高。

Objective To analyze the clinical efficacy of compound clobetasol propionate ointment combined with narrow spectrum ultraviolet in the treatment of mild to moderate psoriasis.Methods A total of 94 patients with mild to moderate psoriasis were selected from May 2020 to April 2021 in Taizhou Second People′s Hospital,which were divided into control group and observation group according to random ball touching method,with 47 cases in each group.The control group was given compound clobetasol propionate ointment,and the observation group was treated with narrow spectrum ultraviolet on the basis of the control group.Both groups were treated for 8 weeks.Clinical efficacy,clinical symptom score(plaque hyper-trophy score,erythema score,scaly symptom score,skin lesion range score),PASI score,TGF-β_(1) level before treatment and after 8 weeks of treatment,incidence of adverse reactions and recurrence rate were compared between the two groups.Results The total effective rate of the observation group was 97.87%,was higher than 82.98%of the control group(χ^(2)=4.423,P=0.035).After 8 weeks of treatment,the scores of plaque hypertrophy,erythema symptom,scaly symptom and skin lesion range in the two groups were lower than those before treatment,and the observation group were lower than those of the control group(P<0.01).After 8 weeks of treatment,the scores of psoriasis area and severity in the two groups were lower than those before treatment,and those in the observation group were lower than control group(P<0.01).After 8 weeks of treatment,the level of serum TGF-β_(1) in the two groups was lower than that before treatment,and that in the observation group was lower than control group(P<0.01).There was no significant difference in the total incidence of adverse reactions and recurrence rate between the two groups(P>0.05).Conclusion Compound clobetasol propionate ointment combined with narrow spectrum ultraviolet have an exact clinical effect for mild to moderate psoriasis,which can effectively improve clinical symptoms of patients,control the degree of skin lesions,reduce the severity of diseases,and with high safety.
