温中散寒法治疗脾胃虚寒胃溃疡的临床研究

Clinical Tudy on Treating Gastric Ulcer of Spleen-stomach Deficiency Cold Type by Warming Middle-jiao and Dispelling Cold

目的:研究温中散寒法治疗脾胃虚寒胃溃疡的临床疗效。方法:选取2021年7月至2021年9月期间于赣州市中医院接受治疗的胃溃疡(脾胃虚寒)患者90例为研究对象。按照随机数字表法将其分为A组,B组,C组各30例。A组予以泮托拉唑钠肠溶片治疗;B组在A组基础上,加以理中汤加减口服;C组患者在B组治疗基础上,加以隔姜灸治疗;三组均治疗8周。比较三组患者治疗前后中医症候积分、胃镜检查观察溃疡变化,治疗结束后随访半年复发及治疗期间的安全性。结果:治疗后三组中医症候积分均低于治疗前,且A组最高、B组次之、C组最低(P<0.05);C组治疗总有效率最高,B组次之,A组最低(P<0.05);A组复发率最高,B组次之,C组最低(P<0.05);三组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:温中散寒法可促进脾胃虚寒胃溃疡患者溃疡面的愈合,促进临床症状改善,降低复发率,且治疗具有较高安全性。

Objective:To study the clinical effect of warming middle-jiao and dispelling cold therapy on gastric ulcer with spleen-stomach deficiency cold type.Methods:From July 2021 to September 2021,90 patients with gastric ulcer(spleen-stomach deficiency cold type)treated in Ganzhou Hospital of Traditional Chinese Medicine were selected as subjects.According to the random number table method,they were divided into group A,group B and group C,30 cases in each group.Group A was treated with pantoprazole sodium enteric-coated tablets,group B was treated with lizhong decoction plus oral administration on the basis of group A,group C was treated with ginger moxibustion on the basis of group B,and the three groups were treated for 8 weeks,respectively.The TCM symptom score and gastroscopy were compared before and after treatment to observe ulcer changes,and the recurrence and safety during treatment were followed up for six months after treatment.Results:After treatment,the TCM syndrome scores of the three groups were lower than before,and the highest in group A,the second in group B and the lowest in group C(P<0.05).The total effective rate in group C was the highest,followed by group B,and the lowest in group A(P<0.05).Group A had the highest recurrence rate,followed by group B and group C(P<0.05).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion:Warming middle-jiao and dispelling cold therapy can promote the healing of ulcer surface,improve the clinical symptoms and reduce the recurrence rate of gastric ulcer patients with spleen-stomach deficiency cold type.
