摘要
目的探讨端粒酶和细胞外基质金属蛋白酶诱导因子(CD147)在肝细胞肝癌(HCC)组织中的表达及其临床病理意义。方法应用组织芯片技术和免疫组化SP法检测272例HCC及81例癌旁组织中端粒酶和CD147的表达情况,并分析其与临床病理特征及预后的关系。结果在272例HCC中端粒酶和CD147的表达阳性率分别为56.62%(154/272)和73.53%(200/272),均高于癌旁组织的9.88%(8/81)和13.58%(11/81),P<0.01。端粒酶在HCC中的表达与术后无瘤生存时间、组织学分级、肿瘤直径、卫星灶、脉管癌栓及临床分期有关,P<0.01,与患者性别、年龄、病灶数目、肝被膜侵犯、肝硬化、血中AFP水平、HBV感染及淋巴结转移无关。CD147的表达与组织学分级、临床分期、术后无瘤生存时间、肿瘤的直径、脉管癌栓相关,P<0.05,与性别、年龄、肝硬化、血中AFP含量、HBV感染、淋巴结转移、病灶数、肝被膜侵犯及卫星灶无关。Kaplan-Meier生存分析结果显示,HCC组织中端粒酶和CD147阳性表达的患者生存时间较阴性表达患者的生存时间缩短。结论HCC中端粒酶和CD147的阳性表达与多种临床病理指标相关,二者均可作为HCC患者预后不良的判断因子。
Objective To investigate the expression of telomerase and CD147 protein in the hepatocellular carcinoma(HCC),and to insight their clinicopathologic significance in HCC.Methods The expression of telomerase and CD147 protein was tested by immunohistochemistry(IHC)in 272 cases of HCC and 81 cases of nocancerous tissue to analyze their relationship between the expression and the clinicopathologic parameters.Results The positive expression rates of telomerase and CD147 in 272 HCC patients were 56.62%(154/272)and 73.53%(200/272)respectively,which were higher than those which were 9.88%(8/81)and 13.58%(11/81)in adjacent noncancerous tissues,P<0.01.The expression of telomerase in HCC was closely correlated with the tumor-free survival time,the histological grade,the diameter of tumor,the microsatellites,the embolus of cancer in aqueduct and TNM clinical stage,P<0.01,and was not correlated with patients′gender,age,the number of tumor,the invasion liver involucrum,liver cirrhosis,the quantity of AFP in blood,the infection of HBV and lymph node metastasis.The expression of CD147 was correlated with the histological grade,the clinical stage,the tumor-free survival time,the diameter of tumor and the embolus of cancer in aqueduct,and was not correlated with the patients′gender,age,liver cirrhosis,AFP level,infection of HBV,lymph node metastasis,number of tumor,invasion liver involucrum and the micro-satellites.The positive expression of telomerase and CD147 were significance related to short survival with Kaplan-Meier survival analysis.Conclusion The positive expression of telomerase and CD147 closely correlates with many clinicopathologic parameters,which may indicate poor prognosis of HCC.
