盆底三维超声对老年女性盆腔器官脱垂的诊断价值探讨 被引量:2

摘要 目的探讨盆底三维超声对老年女性盆腔器官脱垂(POP)的诊断价值。方法选取68例老年女性POP患者作为观察组,另选取68例老年女性非POP患者作为对照组。两组患者均行盆底三维超声检查。对比两组静息状态下、缩肛动作下与Valsalva动作下盆底超声参数。结果静息状态下,观察组膀胱尿道后角与肛提肌裂孔横径、前后径、面积分别为(122.56±11.87)°、(4.75±0.40)cm、(5.56±0.48)cm、(24.42±4.18)cm^(2),均大于对照组的(108.42±12.89)°、(3.98±0.32)cm、(4.45±0.39)cm、(18.18±3.75)cm^(2),差异有统计学意义(P<0.05)。Valsalva动作下,观察组膀胱尿道后角与肛提肌裂孔横径、前后径、面积分别为(148.56±13.65)°、(5.02±0.41)cm、(5.96±0.48)cm、(27.41±4.46)cm^(2),均大于对照组的(123.45±11.75)°、(4.20±0.32)cm、(4.91±0.42)cm、(19.02±3.75)cm^(2),差异有统计学意义(P<0.05)。缩肛动作下,观察组膀胱尿道后角与肛提肌裂孔横径、前后径、面积分别为(120.56±10.75)°、(4.32±0.41)cm、(5.12±0.57)cm、(20.35±3.68)cm^(2),均大于对照组的(106.72±11.48)°、(3.72±0.39)cm、(4.01±0.51)cm、(13.21±2.48)cm^(2),差异有统计学意义(P<0.05)。结论盆底三维超声可观察老年POP患者盆腔器官结构变化,有助于POP早期诊断,指导治疗。 Objective To discuss the diagnostic value of three-dimensional pelvic floor ultrasound for pelvic organ prolapse(POP)in elderly women.Methods 68 elderly female POP patients were selected as the observation group,and 68 elderly female non-POP patients during the same period were selected as the control group.Patients in both groups underwent three-dimensional pelvic floor ultrasound.The pelvic floor ultrasonic parameters under the resting state,anal contraction and Valsalva maneuver were compared between the two groups.Results Under the resting state,the posterior vesicourethral angle,transverse diameter,anteroposterior diameter,area of levator ani muscle hiatus of the observation group were(122.56±11.87)°,(4.75±0.40)cm,(5.56±0.48)cm,and(24.42±4.18)cm^(2),which were greater than(108.42±12.89)°,(3.98±0.32)cm,(4.45±0.39)cm,and(18.18±3.75)cm^(2) of the control group,and the differences were all statistically significant(P<0.05).Under Valsalva maneuver,the posterior vesicourethral angle,transverse diameter,anteroposterior diameter,area of levator ani muscle hiatus of the observation group were(148.56±13.65)°,(5.02±0.41)cm,(5.96±0.48)cm,and(27.41±4.46)cm^(2),which were greater than(123.45±11.75)°,(4.20±0.32)cm,(4.91±0.42)cm,and(19.02±3.75)cm^(2) of the control group,and the differences were all statistically significant(P<0.05).Under anal contraction,the posterior vesicourethral angle,transverse diameter,anteroposterior diameter,area of levator ani muscle hiatus of the observation group were(120.56±10.75)°,(4.32±0.41)cm,(5.12±0.57)cm,and(20.35±3.68)cm^(2),which were greater than(106.72±11.48)°,(3.72±0.39)cm,(4.01±0.51)cm,and(13.21±2.48)cm^(2) of the control group,and the differences were all statistically significant(P<0.05).Conclusion Three-dimensional pelvic floor ultrasound can observe the changes of pelvic organ structure in elderly POP patients,which helps to diagnose POP at an early stage and guide the treatment.
