二甲双胍联合炔雌醇环丙孕酮对多囊卵巢综合征不孕症患者妊娠率的影响分析

Effect analysis of metformin combined with ethinylestradiol and cyproterone acetate on pregnancy rate of patients with infertility due to polycystic ovary syndrome

目的分析多囊卵巢综合征不孕症患者应用二甲双胍联合炔雌醇环丙孕酮治疗对妊娠率的影响。方法80例多囊卵巢综合征不孕症患者,借助方便抽样法划分为对照组及观察组,各40例。对照组采取炔雌醇环丙孕酮治疗,观察组应用炔雌醇环丙孕酮联合二甲双胍治疗。比较两组排卵指标、激素水平、妊娠率及排卵率。结果观察组取卵数量(12.72±2.67)个多于对照组的(10.67±2.04)个,子宫内膜厚度(11.38±2.35)mm厚于对照组的(8.25±1.36)mm,差异有统计学意义(P<0.05)。观察组睾酮(1.05±0.22)mmol/ml、促黄体生成素(2.12±0.32)mIU/L、胰岛素抵抗指数(1.25±0.26)低于对照组的(2.76±0.24)mmol/ml、(2.79±0.76)mIU/L、(2.38±0.42),胰岛素分泌指数(5.67±1.04)高于对照组的(5.12±0.67),差异有统计学意义(P<0.05)。观察组妊娠率87.50%、排卵率92.50%,高于对照组的52.50%、72.50%,差异有统计学意义(P<0.05)。结论多囊卵巢综合征不孕症应用二甲双胍联合炔雌醇环丙孕酮治疗效果显著,对于改善妊娠结局以及临床症状具有积极意义,有利于调整患者代谢状态,安全性比较高,可借鉴以及推广。

Objective To analyze the effect of metformin combined with ethinylestradiol and cyproterone acetate on pregnancy rate of patients with infertility due to polycystic ovary syndrome.Methods A total of 80 patients with infertility due to polycystic ovary syndrome were divided into control group and observation group by convenient sampling method,with 40 cases in each group.The control group was treated with ethinylestradiol and cyproterone acetate,and the observation group was treated with ethinylestradiol and cyproterone acetate combined with metformin.The ovulation indicators,hormone levels,pregnancy rate and ovulation rate were compared between the two groups.Results The number of eggs retrieved(12.72±2.67)eggs in the observation group was more than(10.67±2.04)eggs in the control group;the endometrial thickness(11.38±2.35)mm in the observation group was thicker than(8.25±1.36)mm in the control group;the differences were all statistically significant(P<0.05).The testosterone(1.05±0.22)mmol/ml,luteinizing hormone(2.12±0.32)mIU/L,and insulin resistance index(1.25±0.26)in the observation group were lower than(2.76±0.24)mmol/ml,(2.79±0.76)mIU/L,and(2.38±0.42)in the control group;the insulin secretion index(5.67±1.04)in the observation group was higher than(5.12±0.67)in the control group;the differences were all statistically significant(P<0.05).The pregnancy rate and the ovulation rate of the observation group were 87.50%and 92.50%,which were higher than 52.50%and 72.50%of the control group,and the differences were all statistically significant(P<0.05).Conclusion Metformin combined with ethinylestradiol and cyproterone acetate is effective in the treatment of infertility due to polycystic ovary syndrome,which has positive significance for improving pregnancy outcomes and clinical symptoms,and is conducive to adjusting the metabolic state of patients.It is relatively safe and can be used for reference and promotion.
