当归补血汤加减治疗老年股骨颈骨折患者全髋关节置换术后贫血的疗效分析

Efficacy analysis of modified Danggui Buxue Decoction in the treatment of anemia after total hip arthroplasty for femoral neck fracture in the elderly

目的分析当归补血汤加减治疗老年股骨颈骨折患者全髋关节置换术后贫血的临床疗效。方法选取60例老年股骨颈骨折行全髋关节置换术后贫血患者,采用随机分配法分为中药组与对照组,各30例。对照组采用常规治疗,中药组在对照组基础上采用当归补血汤加减口服治疗。对比两组疼痛程度、髋关节功能评分、治疗效果及红细胞比容(HCT)、血红蛋白(Hb)水平。结果治疗后,中药组Harris评分(88.02±5.63)分高于对照组的(82.40±5.84)分,视觉模拟评分法(VAS)评分(1.26±0.35)分低于对照组的(1.84±0.42)分,差异均有统计学意义(P<0.05)。中药组治疗总有效率96.67%高于对照组的76.67%,差异有统计学意义(P<0.05)。治疗后,中药组HCT、Hb水平分别为(39.47±1.12)%、(120.08±3.15)g/L,均高于对照组的(35.28±1.47)%、(104.70±8.68)g/L,差异均有统计学意义(P<0.05)。结论老年股骨颈骨折患者全髋关节置换术后贫血在常规西药治疗基础上应用当归补血汤加减疗法可优化疗效,改善患者术后贫血症状,促进髋关节功能恢复,值得推广。

Objective To analyze the clinical efficacy of modified Danggui Buxue Decoction in the treatment of anemia after total hip arthroplasty for femoral neck fracture in the elderly.Methods A total of 60 elderly patients with anemia after total hip arthroplasty for femoral neck fracture were randomly divided into traditional Chinese medicine group and control group,with 30 cases in each group.The control group was treated with conventional treatment,while the traditional Chinese medicine group was treated with modified Danggui Buxue Decoction orally on the basis of the control group.The pain level,hip joint function score,therapeutic effect,hematocrit(HCT)and hemoglobin(Hb)levels were compared between the two groups.Results After treatment,the Harris score(88.02±5.63)points of traditional Chinese medicine group was higher than(82.40±5.84)points of the control group,and the visual analogue scale(VAS)score(1.26±0.35)points was lower than(1.84±0.42)points of the control group,and the differences were all statistically significant(P<0.05).The total effective rate 96.67%of the traditional Chinese medicine group was higher than 76.67%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of HCT and Hb in the traditional Chinese medicine group were(39.47±1.12)%and(120.08±3.15)g/L,which were higher than(35.28±1.47)%and(104.70±8.68)g/L in the control group,and the differences were all statistically significant(P<0.05).Conclusion For elderly patients with anemia after total hip arthroplasty for femoral neck fracture,modified Danggui Buxue Decoction on the basis of conventional western medicine can optimize the efficacy,improve patients'postoperative anemia symptoms and promote the recovery of hip function,which is worthy of promotion.
