腹腔镜胆囊手术和开腹胆囊手术并发症的临床比较

Comparative of Clinical Complications of Laparoscopic Gallbladder Surgery and Open Gallbladder Surgery

目的对比腹腔镜胆囊切除术、开腹胆囊切除术应用于胆囊结石伴急性胆囊炎患者治疗价值。方法90例胆囊结石伴急性胆囊炎患者予以随机数字表法分组,研究组行腹腔镜胆囊切除术治疗,对照组行开腹胆囊切除术治疗。比较两组的治疗效果。结果研究组手术时间较长(P<0.05),研究组术中出血量、术后肛门首次排气时间、术后肠鸣音恢复时间、术后引流量、术后住院时间均少于对照组(P<0.05);两组术前IgA、IgG、IgM水平对比,P>0.05,术后对照组上述指标较术前明显下降(P<0.05),研究组手术前后上述指标无显著变化(P>0.05);研究组术后并发症总发生率为8.89%,对照组为26.67%(P<0.05)。结论应用腹腔镜胆囊切除术治疗胆囊结石合并急性胆囊炎患者的安全性较优。

Objective To compare the therapeutic value of laparoscopic cholecystectomy and open cholecystectomy in patients with gallstones with acute cholecystitis.Methods Ninety patients with gallbladder stones and acute cholecystitis were grouped by random number table.The study group was treated with laparoscopic cholecystectomy,and the control group was treated with open cholecystectomy.The effect of the two groups was compared.Results The operation time of the study group was longer(P<0.05).The intraoperative blood loss,the time of the first postoperative anal exhaustion,the recovery time of postoperative bowel sounds,the postoperative drainage volume,and the postoperative hospitalization time of the study group were lesser(P<0.05).The IgA,IgG,IgM levels of the two groups before operation were not statistically significant,P>0.05,the above indexes of the control group after operation were significantly lower than before operation(P<0.05),and the above indexes of the study group had no significant change before and after operation(P>0.05).The total incidence of postoperative complications in the study group was 8.89%,and that in the control group was 26.67%(P<0.05).Conclusion Laparoscopic cholecystectomy is safer for patients with gallstones and acute cholecystitis.
