生化指标中血脂、血糖、血清酶检测对脂肪肝的诊断价值分析

Diagnostic Value Analysis of Lipid,Glucose and Serum Enzyme Tests among Biochemical Indicators for Fatty Liver

目的:探讨生化指标中血脂、血糖、血清酶检测对脂肪肝的诊断价值。方法:选取2017年7月-2019年7月海南省定安县人民医院收治的67例疑似脂肪肝患者设为疾病组,选取同期67例健康体检者设为健康组。检测两组高密度脂蛋白(HDL)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FPG)、碱性磷酸酶(ALP)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)水平,并进行组间比较。分析生化检查与临床病理检查确诊脂肪肝的阳性率以及不同程度脂肪肝患者HDL、FBG、ALP水平。结果:生化检查与临床病理检查确诊脂肪肝的阳性率比较,差异无统计学意义(P>0.05)。疾病组LDL-C、TG、TC水平均高于健康组,HDL-C水平低于健康组,差异有统计学意义(P<0.05)。疾病组FBG、ALT、AST水平均高于健康组,差异有统计学意义(P<0.05)。重度脂肪肝患者HDL、FPG、ALP水平均高于轻度与中度患者,差异有统计学意义(P<0.05)。结论:生化指标中血脂、血糖、血清酶检测对脂肪肝的诊断价值显著,建议推广。

Objective:To explore the diagnostic value of lipid,glucose and serum enzyme tests among biochemical indicators for fatty liver.Methods:Sixty-seven patients with suspected fatty liver admitted to the People's Hospital of Ding'an County,Hainan Province,from July 2017 to July 2019 were selected and set as the disease group.Sixty-seven cases of healthy physical examiners in the same period were selected and set as the healthy group.The levels of high-density lipoprotein(HDL),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),triglycerides(TG),total cholesterol(TC),fasting glucose(FPG),alkaline phosphatase(ALP),alanine aminotransferase(ALT),and aspartate aminotransferase(AST)were measured in both groups and compared between groups.The positive rates of biochemical and clinicopathological examinations to confirm the diagnosis of fatty liver and the levels of HDL,FBG and ALP in patients with different degrees of fatty liver were analyzed.Results:There was no statistically significant difference between the positive rates of biochemical examination and clinicopathological examination to confirm the diagnosis of fatty liver(P>0.05).The levels of LDL-C,TG and TC in the disease group were higher than those in the healthy group,and the levels of HDL-C were lower than those in the healthy group,and the differences were statistically significant(P<0.05).FBG,ALT and AST levels in the disease group were all higher than those in the healthy group,and the difference was statistically significant(P<0.05).HDL,FPG,and ALP levels were higher in patients with severe fatty liver than in those with mild and moderate,and the difference was statistically significant(P<0.05).Conclusion:The diagnostic value of lipid,blood glucose and serum enzyme tests among biochemical indexes for fatty liver is significant and recommended to be promoted.
