摘要 目的:探讨斑点追踪成像(STI)在左室射血分数(LVEF)正常的室性早搏患者射频消融术前后左室收缩功能变化评估中的价值。方法:选取2018年10月-2020年10月深圳市龙岗区人民医院收治的50例室性早搏患者为研究组,另选取同期30例健康体检者设为对照组。研究组接受频消融术治疗,对研究组手术前后、对照组入组时进行常规超声及STI检查。比较两组手术前、研究组手术前后常规超声及STI指标情况。结果:手术前,两组LVEF、左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)、左心室舒张末期内径(LVEDD)水平比较,差异无统计学意义(P>0.05);研究组左心室整体面积峰值应变(GPAS)、左心室整体径向峰值应变(GPRS)、左心室整体环向峰值应变(GPCS)、左心室整体纵向峰值应变(GPLS)水平低于对照组,差异有统计学意义(P<0.05)。研究组手术前后LVEF、LVESV、LVEDV、LVEDD水平比较,差异无统计学意义(P>0.05);术后,研究组GPAS、GPRS、GPCS、GPLS水平高于术前,差异有统计学意义(P<0.05)。结论:STI在LVEF正常的室性早搏患者射频消融术后左室收缩功能评估中具有较高的应用价值,可为临床制定进一步干预方案提供客观参考依据,利于保证疾病治疗效果,促使疾病良好转归。 Objective:To investigate the value of speckle tracking imaging(STI)in the assessment of changes in left ventricular systolic function before and after radiofrequency ablation in patients with premature ventricular beats with normal left ventricular ejection fraction(LVEF).Methods:Fifty patients with ventricular premature beats admitted to the People's Hospital of Longgang District,Shenzhen,China,from October 2018 to October 2020 were selected as the study group.Another 30 cases with healthy physical examination during the same period were selected and set as the control group.The study group was treated with frequency ablation,and routine ultrasound and STI examinations were performed before and after the surgery for the study group and at the time of enrollment for the control group.The conventional ultrasound and STI indexes were compared between the two groups before surgery and before and after surgery in the study group.Results:Before surgery,there was no statistically significant difference in the levels of LVEF,left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),and left ventricular end-diastolic internal diameter(LVEDD)between the two groups(P>0.05).The levels of overall peak left ventricular area strain(GPAS),overall peak left ventricular radial strain(GPRS),overall peak left ventricular circumferential strain(GPCS),and overall peak left ventricular longitudinal strain(GPLS)in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The differences were not statistically significant when comparing the levels of LVEF,LVESV,LVEDV,and LVEDD before and after surgery in the study group(P>0.05);after surgery,the levels of GPAS,GPRS,GPCS,and GPLS in the study group were higher than those before surgery,and the differences were statistically significant(P<0.05).Conclusion:STI has high application value in the assessment of left ventricular systolic function after RF ablation in patients with premature ventricular beats with normal LVEF,which can provide an objective reference basis for clinical development of further intervention plans,which is conducive to ensuring disease treatment effects and promoting good disease regression.
