风险预防精细化护理在股骨骨折内固定术患者围术期的应用

Perioperative value of risk prevention and fine nursing in patients with internal fixation of femur fracture

目的探究手术风险预防精细化护理干预在股骨骨折内固定手术患者中的应用效果。方法选择2019年1月至2020年12月在四川省中西医结合医院收治的98例股骨骨折内固定术患者, 采用随机数字表法分为对照组和观察组, 各49例。对照组实施常规护理干预, 观察组实施手术风险预防精细化护理干预。比较两组患者手术相关指标、平均住院时间、并发症发生率及生活质量。结果观察组患者术中出血量低于对照组, 体温变化较为稳定, 住院时间短于对照组, 差异均有统计学意义(P<0.05)。与对照组相比, 观察组患者术中压力性损伤、切口感染、下肢静脉血栓以及寒战等发生率显著低于对照组, 差异均有统计学意义(P<0.05)。干预前, 两组患者生活质量相比差异无统计学意义(P>0.05);干预后, 观察组患者心理功能、躯体功能、生活状态、社会功能等评分高于对照组, 差异均有统计学意义(P<0.05)。结论对股骨骨折采取内固定手术治疗的患者实施手术风险预防精细化护理干预对策, 在维持患者手术过程中机体各项指标处于正常水平范围的同时, 还可有效降低术中并发症发生率, 提高患者生活质量。

Objective To explore the clinical effect of surgical risk prevention and fine nursing intervention in patients with femur fracture internal fixation.MethodsA total of 98 patients with femoral fracture treated in hospital from January 2019 to December 2020 were randomly divided into control group and observation group,49 cases in each group.The control group received routine nursing intervention.The observation group carried out refined nursing intervention for surgical risk prevention.The operation related indexes,average length of hospital stay,complications and quality of life were compared between the two groups.Results Observation group of patients with intraoperative blood loss was lower than the control group,the temperature change was relatively stable,the hospital stays was shorter than the control group,and the differences were statistically significant(P<0.05).Compared with the control group,the incidence of intraoperative pressure injury,incision infection,lower extremity venous thrombosis and chills in the observation group were significant lower than the control group,and the differences were statistically significant(P<0.05).Before intervention,there was no significant difference in quality of life between the two groups(P>0.05).After the intervention,the observation group of patients psychological function,physical function,life condition,social function score were higher than control group,and the differences were statistically significant(P<0.05).Conclusions The implementation of refined nursing intervention measures for surgical risk prevention in patients with femoral fracture treated with internal fixation can not only maintain the body indicators in the normal range during the operation,but also effectively reduce the incidence of intraoperative complications and improve the quality of life of patients.
