摘要
目的探讨男方年龄的差异及精液参数的不同对体外受精(in vitro fertilization,IVF)结局的影响。方法对556例IVF周期各项临床数据进行回顾性分析,探究精液质量与IVF结局的相关性,并将各周期按照男性年龄分组,通过精子浓度、精子总活动力及前向运动率(progressive rate,PR)等数据,反映男性年龄对男性精液质量的影响;通过受精率、卵裂率、可移植胚胎率及优质胚胎率等数据,反映男性年龄对受精情况及早期胚胎发育情况的影响。结果(1)男性精子浓度、总活动力及前向运动率与可移植胚胎率显著性正相关。(2)男性40岁以后精子浓度、精子总活动力及前向运动率均显著下降,其IVF周期内受精率、可移植胚胎率及优质胚胎率也显著降低。结论男性精液质量与IVF周期受精结局具有相关性,男性年龄通过影响精液质量间接负调IVF周期受精与早期胚胎发育情况。
Objective To explore the effects of male age and semen quality on the outcomes of IVF.Methods Aretrospective analysis of clinical data of 556 IVF cycles was carried out to explore the correlation between semen quality and the outcome of IVF.Then each cycle was grouped according to male age.The effect of male age on semen quality was reflected through data such as sperm concentration,total sperm motility rate and forward movement rate.The effect of male age on fertilization and early embryo development was evaluated through data such as fertilization rate,cleavage rate,transferable embryo rate and high-quality embryo rate.Results Male sperm concentration,total motility rate and forward movement rate were significantly positively correlated with the rate of transferable embryos.For the patients above 40 years old,the sperm concentration,total sperm motility rate,and forward motility rate were decreased significantly,and fertilization rate,transferable embryo rate and high-quality embryo rate also declined significantly.Conclusion Male semen quality is significantly correlated to fertilization outcome.Male age indirectly negatively regulates fertilization and early embryonic development by affecting semen quality.
