胃癌患者多层螺旋CT影像学表现与临床特征的关系

Relationship of multi-slice spiral CT imaging with clinical features of patients with gastric cancer

目的探讨胃癌患者多层螺旋CT影像学表现与临床特征的关系。方法选取60例胃癌患者,均采用多层螺旋CT检查,以术后病理检查结果作为诊断金标准,分别对胃癌的TNM分期进行分析,并分析多层螺旋CT影像学表现与临床特征的关系。结果多层螺旋CT检查诊断胃癌T分期的准确度为86.67%(52/60),诊断N分期的准确度为93.33%(56/60),诊断M分期的准确度为85.00%(51/60)。T_(3~4)期胃癌患者肿瘤最大径明显大于T_(1~2)期患者,N_(1~2)期胃癌患者强化程度、瘤周低密度带厚度、肿瘤最大径及肿瘤厚度均明显大于N_(0)期患者,M1期胃癌患者强化程度明显大于M_(0)期患者,未分化型胃癌患者强化程度、瘤周低密度带厚度、肿瘤最大径均明显大于分化型患者,差异均有统计学意义(P﹤0.01)。结论采用多层螺旋CT对胃癌患者进行检查,可及时明确患者的病情分期,同时还能根据患者的CT影像学表现评估其疾病严重程度,临床应用价值较高。

Objective To investigate the relationship of multi-slice spiral CT imaging with clinical features of patients with gastric cancer.Method A total of 60 patients with gastric cancer who were examined by multi-slice spiral CT were selected.The TNM staging of gastric cancer,and the relationship between the imaging of multi-slice spiral CT and clinical features were analyzed by using postoperative pathological result as the gold standard.Result The accuracy of multi-slice spiral CT in diagnosing gastric cancer T staging was 86.67%(52/60),that of N staging was 93.33%(56/60),and that of M staging was 85.00%(51/60).The maximum tumor diameter of T_(3-4) staging was significantly higher than that of T_(1-2) staging;the enhancement degree,thickness of peritumoral low density band,maximal tumor diameter and thickness of the N_(1-2) staging were significantly higher than those of N_(0) staging;the enhancement degree of the M1 staging were significantly higher than that of M_(0) staging;the enhancement degree,thickness of peritumoral low density band and maximal tumor diameter of the undifferentiated type were significantly higher than those of differentiated type(P<0.01).Conclusion Multi-slice spiral CT in the examination of gastric cancer can timely determine the staging of disease,and the severity of the disease can be evaluated according to the CT imaging manifestations of the patients,which has higher clinical application value.
