慢性颈痛患者颈肩肌的表面肌电信号特征分析 被引量:2

摘要 目的:通过评估慢性颈痛患者颈肩部肌肉功能状态,为颈痛患者合理预防与康复治疗提供理论依据。方法:选取20例慢性颈痛患者为试验组及10名年龄、性别等相匹配的健康者作为正常对照组。记录所有受试者肩关节外展90°做最大等长收缩状态下双侧上斜方肌和三角肌中束的sEMG信号,选取RMS和MF两个指标进行统计分析。结果:正常对照组双侧上斜方肌、三角肌中束的RMS、MF值差异无统计学意义(P>0.05);试验组双侧上斜方肌、三角肌中束的RMS、MF值小于正常对照组(P<0.05);试验组疼痛侧三角肌中束的RMS值和MF值均小于非疼痛侧(P<0.05);试验组疼痛侧上斜方肌的RMS值小于非疼痛侧,差异有统计学意义(P<0.05);疼痛侧上斜方肌的MF与非疼痛侧相比,差异无统计学意义(P>0.05)。结论:慢性颈痛患者不仅出现颈背部肌肉活动水平的下降,还会出现肩周肌肉活动水平的下降,在临床治疗慢性颈痛患者的过程中,不能忽视对肩周肌肉及肩关节功能的训练。 Objective:To evaluate the neck-shoulder muscles function in patients with chronic neck pain,and provide theoretical basis for the reasonable prevention and rehabilitation of chronic neck pain.Methods:Twenty patients with chronic neck pain and 10 age-and gender-matched healthy participants were recruited for the present study.Surface Electromyographic(sEMG)activity was recorded from middle deltoid and upper trapezius during maximum isometric voluntary contraction for shoulder joint abduction 90 degrees.Root mean square(RMS)and median frequency(MF)were compared and analyzed.Results:There were no significant differences in the value of either RMS or MF between the left and right sides of healthy subjects(P>0.05).Significant differences existed in RMS and MF between chronic neck pain patients and controls(P<0.05).The value of RMS and MF of middle deltoid on the patients'pain side were significantly lower than those of the non-pain side(P<0.05).The value of RMS of upper trapezius on the patients’pain side were significantly lower than that of the non-pain side(P<0.05).But there was no significantly difference of MF of upper trapezius between the patients'pain side and non-pain side.Conclusions:Chronic neck pain can affect the shoulder muscle activity.During the course of clinical treatment of chronic neck pain,shoulder muscle and shoulder joint function cannot be ignored.
