摘要
目的探究维生素C对冠心病经皮冠状动脉介入术(PCI)术后患者炎性损伤、心肌纤维化及分形趋化因子(Fractalkine)、内脏脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)、高迁移率族蛋白(B1HMGB1)的影响。方法回顾性分析2018年6月至2021年8月在陕西中医药大学第二附属医院心内科接受PCI术治疗的160例冠心病患者的临床资料,依据治疗方式进行分组,其中80例术后进行常规对症治疗的患者纳入对照组,80例在术后常规治疗的基础上联合维生素C治疗的患者纳入观察组。比较两组患者治疗前后的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、Ⅰ型前胶原(PCⅠ)、Ⅲ型前胶原(PCⅢ)、半乳糖凝集素-3(Gal-3)、胎盘生长因子(PLGF)、分形趋化因子(Fractalkine)、内脏脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)、高迁移率族蛋白B1(HMGB1)水平。结果治疗后,观察组患者的血清TNF-α、IL-6分别为(60.41±9.63)pg/mL、(29.72±5.04)pg/mL,明显低于对照组的(65.02±14.30)pg/mL、(34.01±4.78)pg/mL,差异均有统计学意义(P<0.05);治疗后,观察组患者的血清PCⅠ、Gal-3、Fractalkine、HMGB1分别为(162.44±15.8)ng/mL、(3.67±0.64)ng/mL、(483.74±47.10)ng/L、(4.98±0.92)μg/L,明显低于对照组的(167.17±13.21)ng/mL、(3.92±0.86)ng/mL、(506.54±49.55)ng/L、(5.29±0.84)μg/L,差异均有统计学意义(P<0.05);而治疗后,两组患者的血清PCⅢ、PLGF、Vaspin比较差异均无统计学意义(P>0.05)。结论维生素C联合常规PCI术后冠心病二级预防治疗可有效减轻冠心病PCI术后炎性损伤,改善其心肌纤维化,优化Fractalkine、B1HMGB1因子,有利于降低炎症反应。
Objective To explore the effects of vitamin C on inflammatory injury,myocardial fibrosis,Fractalkine,Visceral adipose-specific serine protease inhibitor(Vaspin),and high mobility group box 1 protein(HMGB1)in patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods The clinical data of 160 patients with coronary heart disease who underwent PCI in Department of Cardiology,the Second Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from June 2018 to August 2021 were analyzed retrospectively.According to the treatment method,80 patients who received conventional symptomatic treatment after surgery were included in the control group,and 80 patients who received vitamin C combined with conventional symptomatic treatment after surgery were included in the observation group.The levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),typeⅠprocollagen(PCI),typeⅢprocollagen(PCⅢ),galactose lectin-3(Gal-3),placental growth factor(PLGF),Fractalkine,Vaspin,and HMGB1 before and after treatment in the two groups were compared.Results After treatment,the serum TNF-αand IL-6 in the observation group were(60.41±9.63)pg/mL and(29.72±5.04)pg/mL,which was significantly lower than(65.02±14.30)pg/mL and(34.01±4.78)pg/mL in the control group(P<0.05).After treatment,the levels of PCⅠ,Gal-3,Fractalkine,and HMGB1 in the observation group were(162.44±15.8)ng/mL,(3.67±0.64)ng/mL,(483.74±47.10)ng/L,and(4.98±0.92)μg/L,which were significantly lower than(167.17±13.21)ng/mL,(3.92±0.86)ng/mL,(506.54±49.55)ng/L,and(5.29±0.84)μg/L in the control group(P<0.05).After treatment,there was no significant difference in serum PCⅢ,PLGF,and Vaspin between the two groups(P>0.05).Conclusion Vitamin C combined with conventional symptomatic treatment for coronary heart disease after PCI can effectively reduce the inflammatory injury after PCI,improve myocardial fibrosis,and optimize Fractalkine and B1HMGB1 factors,which is conducive to reducing inflammatory response.
