甲状腺激素水平检测在老年抑郁症患者中的应用价值分析

Analysis of the Application Value of Thyroid Hormone Level in Elderly Patients with Depression

目的:探讨检测血清甲状腺激素水平在老年抑郁症中的应用价值。方法:本研究的老年抑郁症患者均为2018年1月—2020年1月收治,共计86例,根据抑郁程度不同将其分为3组:轻度抑郁组(43例)、中度抑郁组(28例)、重度抑郁组(15例)。选择2020年6月—2020年7月期间来我院健康体检的43例健康志愿者作为对照组。采用电化学发光法检测促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)。结果:与对照组比较,观察组血清T3、FT3及FT4水平降低,TSH水平升高(P<0.05)。随着抑郁程度加重,血清T3、FT3及FT4水平逐渐减低,TSH水平逐渐升高(P<0.05)。相关性分析结果显示,老年抑郁症患者HAMD评分与血清T3、FT3及FT4水平均呈负相关,与血清TSH水平呈正相关(P<0.05)。结论:老年抑郁症患者血清甲状腺激素水平分泌存在异常,并于抑郁症的发生、发展以及病情严重程度有着密切关系。

Objective:To investigate the value of serum thyroid hormone level in elderly depression.Method:The elderly patients with depression in this study were all admitted from January 2018 to January 2020,with a total of 86 cases.They were divided into three groups according to the degree of depression:mild depression group(43 cases),moderate depression group(28 cases),and severe depression group(15 cases).43 healthy volunteers who came to our hospital for physical examination from June 2020 to July 2020 were selected as the control group.Thyroid stimulating hormone(TSH)、Free triiodothyronine(FT3)、Free thyroxine(FT4)、triiodothyronine(T3)and thyroxine(T4)were detected by electrochemiluminescence method.Result:Compared with the control group,the levels of serum T3,FT3 and FT4 in the observation group decreased,and the levels of TSH increased(P<0.05).With the aggravation of depression,the levels of serum T3,FT3 and FT4 gradually decreased,and the level of TSH gradually increased(P<0.05).The results of correlation analysis showed that HAMD score in elderly patients with depression was negatively correlated with serum T3,FT3 and FT4 levels,and positively correlated with serum TSH levels(P<0.05).Conclusion:The abnormal secretion of serum thyroid hormone in elderly patients with depression is closely related to the occurrence,development and severity of depression.
