身痛逐瘀汤加减+艾瑞昔布片治疗椎体压缩性骨折PVP术后残留痛(血瘀证)的临床治疗效果

Clinical Effect of Modified Shentong Zhuyu Decoction + Erecoxib Tablets in the Treatment of Residual Pain(Blood Stasis Syndrome)After PVP of Vertebral Compression Fractures

目的:分析身痛逐瘀汤加减+艾瑞昔布片治疗椎体压缩性骨折PVP术后残留痛(血瘀证)的临床治疗效果。方法:选取2020年1月—2021年12月我院收治的椎体压缩性骨折PVP的患者102例为研究对象,以抽签方法分为对照组和观察组,各51例。对照组PVP术后采用艾瑞昔布片治疗,观察组在对照组基础上联合身痛逐瘀汤加减,比较两组腰椎功能障碍改善程度、疼痛程度、血瘀证评分、炎症因子。结果:术后7 d,观察组Oswestry功能障碍指数(ODI)低于对照组(P<0.05);观察组术后12 h、1 d、3 d、7 d疼痛数字评分(NRS)低于对照组(P<0.05);观察组术后3、7 d血瘀证评分低于对照组(P<0.05);观察组术后1、3、7 d TNF-α、IL-6、CRP水平低于对照组(P<0.05)。结论:身痛逐瘀汤加减联合艾瑞昔布片治疗椎体压缩性骨折PVP术后残留痛(血瘀证)效果显著,可改善患者的腰椎功能,减轻术后残余疼痛和血瘀症状,降低炎症反应,具有应用有效性。

Objective: To analyze the clinical therapeutic effect of modified Shentong Zhuyu Decoction + erecoxib tablets in the treatment of residual pain(blood stasis syndrome) after PVP for vertebral compression fractures. Methods: A total of 102 patients with vertebral body compression fracture PVP treated in our hospital from January 2020 to December 2021 were selected as the research objects, and divided into control group and observation group by drawing lots, with 51 cases in each group. The control group was treated with erecoxib after PVP, and the observation group was combined with Shentong Zhuyu Decoction on the basis of the control group, and the improvement degree of lumbar spine dysfunction, pain degree, blood stasis syndrome score and inflammatory factors were compared between the two groups. Results: On the 7th day after operation, the Oswestry disability index(ODI) in the observation group was lower than that in the control group(P<0.05);the pain numerical score(NRS) of the observation group at 12 h, 1 d, 3 d, and 7 d after operation was lower than that of the control group(P<0.05);the blood stasis syndrome score of the observation group was lower than that of the control group at 3and 7 days after operation(P<0.05);the levels of TNF-α, IL-6 and CRP in the observation group were lower than those in the control group at 1, 3 and 7 days after operation(P<0.05). Conclusion: Shentong Zhuyu Decoction combined with erecoxib tablets has a significant effect on residual pain(blood stasis syndrome) after PVP of vertebral body compression fractures,which can improve the patient’s lumbar spine function, reduce postoperative residual pain and blood stasis symptoms, reduce inflammatory response, and has application effectiveness..
